The registered nurse shall render behavioral health home services and nurse delegation to clients who live in licensed assisted living facilities, community group homes and in their own homes. Some clients who reside in their own homes may opt to be serviced in the office during therapeutic group times.
The registered nurse shall report to the Psychiatric Rehabilitation Program Director regularly and service each client in the caseload face to face once per month. Clients on the caseload may reside in multiple locations and the nurse may not necessarily have a designated office workspace while working in the community.
Depending upon caseload, the nurse shall be required to render 15 - 25 hours per week in direct treatment services. No compensation is paid outside of services listed on each service invoice. The nurse shall be paid on a fee for service basis and a pre-filled invoice must be submitted to the Director for each service rendered. The fee paid to the nurse for each invoice submitted is $35.00. All services on the invoice must be completed in order to receive compensation for the service invoice. Compensation is paid bi-weekly. The nurse is expected service at least 1 client per hour.
When servicing a client, all services on the invoice must be completed. The list of services for each invoice is as follows:
1. Review, print, sign, date and attach to the white and yellow copies of this invoice the CRISP
PDMP report since last RN/CM/DN visit (prescriptions for all Maryland prescribers). Complete a medical records request form for any new prescribers external to CWC since the last RN/CM/DN visit and attach medical record request to the white and yellow copies of this invoice.
2 Review, print, sign, date and attach to the white and yellow copies of this invoice the CRISP
snapshot report (hospitalizations, provider notes, discharge orders for all Maryland prescribers)
since last RN/CM/DN visit. Complete a medical records request form for any new facilities/providers external to CWC since the last RN/CM/DN visit and attach medical record request to the white and yellow copies of thisinvoice.
3 Review, print, sign, date and attach to the white and yellow copies of this invoice the ICANotes
clinical order sheets issued by somatic OMHC prescriber (Starlene) since last RN/CM/DN visit.
4 Review, print, sign, date and attach to the white and yellow copies of this invoice the ICANotes
clinical order sheets issued by psychiatric OMHC prescriber (Dr. Warren) since last RN/CM/DN
5 Ensure all medication prescriptions and treatment orders found in items 1-4 reflect in the
iCAREMANAGER medication list; update iCAREMANAGER with any missing medication/treatment orders at this time.
6 Review the medication cart space designated for the client’s medication with the assigned CMT
present to verify compliance with 3-way check and CDS medication security standards.
7 Review the client’s MAR with the assigned CMT present to verify CMT and client’s medication
compliance with order. Issue the CMT written supervision order as necessary and add copy to file
8 Collaborate with pertinent parties of the treatment team for any medication / treatment issues as
necessary. (i.e. troubleshoot with the pharmacy any missing medications ordered by a prescriber
for which you have an order)
9 Complete a RAT in iCAREMANAGER. Complete a self medication readiness assessment form and attach to the white and yellow copies of this invoice.
10 If necessary, based on the results of the above listed items 1-9, update in iCAREMANAGER, the
most recent assisted living service plan and direct care staff routine care tracker tasks. Brief direct care team on emergent changes that cannot wait until this months’ training session.
11 Document the two OMHC evaluation (RAT and SMRA) completed in item 9 on ICANotes using
the appropriate components as a single 99211 nursing note.
12 Document the 11 BHH services on ICANotes as a single W1761 nursing note.
13 Review, update (if necessary) and sign the most recent ICANOTES ITRP.
Other responsibilities include (compensated on an invoiced hourly basis as needed and pre-approved by the supervisor):
- Taking ownership in the implementation / monitoring infection control protocols in the facilities in which you service clients
- Participating and upholding CARF and COMAR standards as it pertains to your scope
- Supervising medication technicians
- Collaborating with ALM, AALM, cook, direct care staff, other members of treatment team, and external entities regularly as it pertains to client care
