Older Adult Service Team (OAST) and Flexible Subsidy Housing Program

administrative offices in San Francisco and Alameda Counties The Senior Division specializes in a full continuum of older adult mental health outpatient programs and a range of aging services programs, including workforce development, economic security, case management, and advocacy. The Division is also very active with community partners and the development of new service areas and policy recommendations
 

Program Description
Felton’s Older Adult Service Team, based in Alameda County, offers specialized, multidisciplinary geriatric mental health services and other aging-related social services to a diverse senior population. Our comprehensive mental health services include medication assessment and management, care management, individual and group therapy, community integration, and specialized outreach in the community. The clinical team includes a psychiatry, clinical case managers, and peer/community specialists. We serve people living
with severe mental illness throughout Alameda County. The Flexible Subsidy Housing Program based in San Francisco County provides case management services and support to clients who are settling into their residence and require support in their path to recovery, including housing retention. The Goal of the Program is to provide culturally responsive, client-centered services, which respond to the needs of clients. Our experience includes outreaching to and engaging clients, problem solving barriers with providers and housing supports, medication support services/monitoring, and navigating viable alternatives when resources expire, or eligibility is not maintained.

Job Description
The Licensed Vocational Nurse exercises medical responsibilities for psychiatric evaluation, medication assessment and medication monitoring, as part of the treatment team. The LVN assists with crisis interventions, including 5150 evaluations, as needed, orders diagnostic
tests, and communicates medical issues with Primary Health Care provider. The LVN maintains client caseload and clinical records in accordance with mandated standards, consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care.
This position works with two programs, one based in Alameda County, the other in San Francisco County. The provider’s schedule will include four days in Alameda County with the Older Adult Service Team and one day in San Francisco with the Flexible Subsidy Housing
Program.

Program Specific Responsibilities:

  • Exercises medical responsibilities for psychiatric evaluation, medication assessment, and medication monitoring
  • Orders diagnostic tests as needed
  • Works with an interdisciplinary team that includes a clinical case managers, peer/ community specialists, and other allied health professionals in the community
  • Works in collaboration with the clinical team to provide therapy and education to clients, including but not limited to harm reduction, relapse prevention, and motivational interviewing
  • Works in collaboration with the clinical team to make appropriate referrals and linkages to other services in support of clients.
  • Coordinates medical issues with Primary Health Care provider • Maintains client caseload, as needed
  • Maintains clinical records in accordance with mandated standards, including conducting psychiatric diagnostic evaluations.
  • Consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care

Minimum Qualifications:

  • Completion of training in an accredited Licensed Vocational Nurse Program 
  • Active Licensed Vocational Nurse license from the California Board of Licensed Vocational Nurses, or ability to obtain one with three months of employment
  • Active Licensed Vocational Nurse Furnishing Number Experience in working with geriatric population
  • Demonstrated commitment to the welfare of individuals and families with mental/ behavioral health issues
  • Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases and psychosis. This includes symptoms, treatment alternatives, drug properties and interactions and preventative healthcare measures
  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders
  • Complex problem solving; identifying complex problems and reviewing related information to develop and evaluate options and implement solutions
  • Critical thinking skills; using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems
  • Social perceptiveness skills; being aware of others' reactions and understanding why they react as they do.
  • Service orientation skills; actively looking for ways to help people
  • Demonstrated interest in ongoing learning and training
  • Ability to be a team player with a welcoming attitude
  • Valid California Driver’s license and car required
