Job Description

 

Responsibilities: This position is community based and TEAM oriented working with the SPMI population providing intensive case management with a therapeutic approach to individuals that are in need of independent housing by providing interventions to individuals in all housing settings to maintain tenancy, support housing transitions and prevent homelessness.  Provides clinical services to support individuals to obtain and/or maintain stability that best meets the individual’s needs and preferences and supports treatment goals.

Qualifications: Bachelor's Degree in Human Service field, such as Social Work, Psychology, Rehabilitative Counseling, or closely related discipline is required.   ACT team experience a plus.

Schedule:  8am-4pm on call and holidays are required.
