Purpose of the position: Responsible for supervising LADC's and to provide comprehensive clinical services and care management for individuals suffering from SUD and co-occurring disorders at the Granite Pathways Hub.

Central Responsibilities: Provide supervision for LADC's in the Manchester and Nashua Hubs. Perform holistic psychosocial and diagnostic assessment consistent with ASAM criteria, develop collaborative treatment plan with individuals. Maintain communication with the individuals, their families and other key community supports for robust aftercare planning.

  • Work cooperatively with other staff as a member of the interdisciplinary team;
  • When indicated, perform face-to-face assessment of individuals to determine the nature and severity of SUD and/or co-occurring conditions consistent with ASAM guidelines and level of care criteria;
  • Orient participants to program goals and objectives, opportunities for growth and requirements for participation, participant rights and overall program operations;
  • Demonstrate a person-centered approach to treatment planning and clinical intervention;
  • Conduct ongoing assessment of individuals utilizing the GPRA tool;
  • Ensure the maintenance of accurate, complete, timely and high-quality documentation and data collection to meet best practice clinical standards that comply with program policies and procedures, payor requirements and external regulatory standards;
  • Participate in case conferences with other community-based service providers;
  • Provide outreach/education/prevention services to the local community, schools, groups, and agencies as necessary and as approved by management staff;
  • Provides crisis intervention/management, generating referrals to other provider resources, as indicated

  • Training in SUD and co-occurring treatment;
  • Knowledge of community resources to treat SUD, emotional and behavioral problems, enhance educational success, secure employment and housing services;
  • Knowledge, commitment and ability to embrace agency mission and philosophy and adhere to policies and procedures;
  • Knowledge of and ability to comply with regulatory and payor requirements and standards;
  • Highly organized and detail-oriented;
  • Excellent oral, written and interpersonal communication skills;
  • Computer literate with a working knowledge of MS Word and MS Excel;
  • Strong writing skills; competencies in writing goal based/person centered treatment plans and progress notes as evidenced by successful completion of writing sample; and
  • Ability to participate in CPR, Naloxone training and First Aid training


  • License eligible master's degree in psychology, counseling, social work or related field OR MLADC
  • Licensed Clinician
  • Multi-lingual clinician preferred


  • One (1) year work experience in treatment of SUD and/or co-occurring disorders
  • Minimum three (3) years of experience in case management or comparable position
  • Experience in working within a 12-step framework
  • Experience working with families affected by the disease of addiction
  • Must be over the age of twenty-one (21) years old
