Position Summary:
The MOMS Home Visitor performs regularly scheduled home visits for families with children ages birth to eight years old. Home Visits are conducted weekly or biweekly, focusing on child development, strengthening parenting skills, and promoting a positive parent child relationship. In addition, the home visitor provides case management to families as needed, and helps connect families with referrals to other community resources.
In addition to home visiting, the position is responsible for assisting the promotion and execution of ongoing group programming for clients. The MOMS Home Visitor performs outreach in the community to identify and recruit new clients.
Essential Functions and Responsibilities:
- Provide home visiting and case management services to a caseload of 10 to 12 families living in the Greater Norwalk Area.
- Facilitate groups and workshops as assigned; assist with transportation and/or childcare to groups/events as needed.
- Outreach to identify and recruit new clients for the MOMS Program
- Connect clients with appropriate referrals to community resources, including health and social services
- Accompany clients to medical and agency appointments as necessary
- Serve as an advocate for clients with schools, health care, and agency providers
- Provide accurate and relevant information on child development and child health and wellness
- Identify client goals and work towards the attainment of client goals
- Assist client in strengthening parenting skills and problem-solving skills
- Help clients identify and use their informal support network
- Participate in orientation and training sessions
- Attend regularly scheduled supervision and staff meetings
- Attend community and state-wide meetings and trainings as assigned
- Complete all assigned paperwork in an accurate and timely manner
- Maintain client records in online database
Essential Qualifications/Skills:
- Must have awareness of cultural and socioeconomic characteristics of clients served.
- Bilingual – Spanish/English
- Ability to work as a team
- Knowledge and expertise in the areas of child development, parenting, community resources, outreach, prenatal/post partum health care, and diverse populations
- Bachelors preferred; Associate’s degree in human services, social work, early childhood education or related discipline required
- Experience facilitating groups preferred
- Must hold valid Connecticut drivers license
- Must have reliable transportation
Physical Demands:
- Must be able to climb several flights of stairs
- Must be able to work with clients in a variety of settings
Working Conditions and Environment:
- Evening and weekend hours as needed
- Must hold valid Connecticut driver’s license and be willing to drive to home visits within the community and collaborative meetings and trainings within the state
- Ability to be flexible with work schedule
