Overview

Family Practice Associates was founded by Dr. Jeffrey Foxx in 1983 and has grown to one of the largest family practice groups in Central Kentucky. Today FPA is a group of primary care providers who are dedicated to giving family-centered care from birth to later years. FPA has over 20 primary providers including: physicians, nurse practitioners, physician assistants, physical therapists, and a marriage and family therapist. With a professionally trained staff of CMA’s, receptionists, lab techs and others, FPA provides award winning care that has been recognized as a certified Patient-Centered Medical Home (PCMH) through the National Commission on Quality Assurance. Responsibilities

The Population Health & Medicare Wellness RN OR LPN (Full-time) will be at our Brannon Crossing location and will be involved in direct patient care as well as administrative tasks. In summary, responsibilities include scheduling and performing Medicare Annual Wellness Visits, coordinating patient care, patient education, and communicating with patients, providers, and care teams.

  • Works collaboratively with providers, care teams and colleagues to schedule and perform Medicare Annual Wellness Visits for patients including history taking, counseling, instruction, and documentation.
  • Works with patients and families on self-management support to create appropriate care plans and goals according to the patient’s needs and conditions.
  • Educates and coaches patient on treatment plans and behavior modifications to meet goals.
  • Manages various aspects of patient care including referrals, documentation, follow-up on admissions and ER visits, ancillary testing, patient education, etc.
  • Coordinates continuity of care with external healthcare organizations and facilities, including hospital admissions and discharges.
  • Communicates with patients regarding care plans, treatment, referrals, follow-up appointments and access to community resources.
  • Reviews and generates reports as necessary from hospitals and managed care company programs to identify gaps in care and clinical conditions needing appropriate follow-up.
  • Performs pre-visit planning, as able, to ensure appropriate follow-up and documentation of targeted conditions including health maintenance items.
  • Proactively acts as patient’s advocate, working to resolve patient concerns.
  • Ensures that documentation and billing of services performed is sufficient to meet CMS guidelines, correct coding guidelines and other payer quality incentive program guidelines.
  • Understands population health management and works to ensure that Medicare and other applicable patients are managed within PCMH & ACO program guidelines.
  • Works with Population Health Director, Administrator, Clinical Manager and Providers to develop clinical protocols.
  • Maintains accurate documentation of conditions, communication, and follow-up care in EHR.
  • Maintains confidentiality of patient information according to established practice policies and governmental regulations, such as HIPAA.
  • Participates in committee and staff meetings as required.

Specific Requirements

  • Registered Nurse (RN) or Licensed Practitioner Nurse (LPN) – Associates or bachelors’ degree in nursing. Current licensure, as appropriate, in the state of Kentucky.
  • Minimum of 2-5 years previous experience in a clinical setting, experience with an electronic health record preferred.
  • Knowledge of primary care medicine including examination, treatment procedures, etc. Experience with patients in the Medicare population preferred.
  • Knowledge of clinical standards, performance improvement, quality assurance and reporting. Previous experience with PCMH, MU or other applicable programs preferred.
  • Proficient in Word, Excel and other computer programs.
  • Demonstrate sound judgment, problem solving and critical thinking skills.
  • Ability to work independently and collaborate with other team members.
  • Excellent communication skills, both written and verbal. Ability to listen well and work closely with patients, staff and providers.
  • Physical ability to sit, stand and stretch repeatedly as well as lift loads of less than 10 pounds.
