Success Coach - Innovative Family Services - FYI+ Main Office

Families and Youth. Inc. is hiring a full-time Success Coach

Summary of Duties: This position provides direct case coordination services to assigned families and/or young adults who are potentially involved in multiple systems to include: probation, protective services, treatment programs, court/legal systems, at-risk prevention/intervention services, etc. Other services provided by this position include service plan development, transition/aftercare coordination, life skills development, psychoeducational services, career and educational developments, restorative justice, family engagement, therapeutic programming, and more. All services provided are done so with a Trauma-Informed / Attachment Healing approach and philosophy. This position requires some travel and occasional overnight program activities. BIPOC and LGBTQ+ individuals with lived experiences are encouraged to apply.

  • Provide clients with supportive services
  • Coordinate services between providers and clients
  • Interpret agency services to clients and community
  • Provide case management services and skill-building services to youth in the program
  • Provide housing case management services
  • Maintain individual case records on clients receiving services
  • Provide specialized case management services to individual client needs
  • Educate clients on available community resources and services
  • Create programs and services to benefit clients and the community
  • Attend community meetings relevant to program and clients
  • Administer and facilitate the completion of appropriate intake forms, assessments, intake packet, and service plans
  • Conduct field visits, attend meetings and be available to deliver services during traditional and non-traditional hours (when needed) in; the client's home, educational institutions, places of employment, FYI's Main office, JPPO offices, detention facilities, reintegration centers, or any other setting that enables and promotes, the consistent and structured delivery of LVP services
  • Organize and facilitate Life Skills Groups, as needed, in both individual and group settings
  • Required to participate and interact in youth activities (for example: camping, sport activities, overnight youth conferences, etc.), which may involve occasional overnight activities and trips


  • Bachelor's degree in Social Work, Psychology, Criminal Justice or Social Services related field and a minimum of one (1) year experience working with at risk youth, young adults, and/or families.
  • Experience working with BIPOC,
  • Must be able to pass a CYFD Background Check.
  • Spanish/English bilingual is also preferred but not required.
