Unity Point Health - Eyerly Ball is seeking a full-time, non-exempt Service Coordinator in our Des Moines location.

As a Service Coordinator, you will partner with adults with serious mental health to provide a quality coordination program, focusing on the individual's strengths and desires. In this fast paced environment, the Service Coordinator assesses the client's needs to develop and implement an individualized service plan designed to improve the client's overall health and quality of life.

As a Service Coordinator you will:

  • Provide outreach activities to support and engage clients in service coordination.
  • Conduct individualized, comprehensive person-centered assessments.
  • Coordinate treatment with a multi-disciplinary team.
  • Advocate for the client's family members, including recommendation of support services.
  • Work with Regions to verify continued client eligibility and funding.

Ideal Candidate:
  • Capacity to professionally relate to a wide variety of clients, as well as a desire to serve as a client advocate.
  • Ability to practice good verbal and written communication skills with ability to complete case notes timely and other required documentation.
  • Capacity to engage with clients effectively, patiently and respond professionally at all times.
  • Ongoing passion about making a difference in the lives of those who have been touched by mental illness.

To be considered you'll need:
  • Bachelor's degree in social work, psychology, human services or related field.
  • One year experience working with population served.
  • Possess a valid driver's license.
  • Required to provide proof of insurance.

Schedule:

Monday-Friday, 8-4:30 PM

COVID-19 and Influenza Vaccines:
