Job Description:
Eternity Homecare Inc. in Madison is looking for a client-service-focused Client Care Coordinator and Human Resources Assistant. They will gather pertinent patient/client, clinical and financial information to optimize sales, customer responsiveness and satisfaction, and quality care.
Essential Functions: Telephone liaison for referral sources, outside agencies, health professionals, and field staff Provides patients/clients and referral sources with crucial information to assist in accessing appropriate services Completes intake screening including obtaining, documenting, and analyzing all required information to make a preliminary admission decision Communicates with patient/client and family regarding arrangements for the initiation of care/services Serve On-Call to address client and staff questions as well as ensure clients receive care. Accompanies caregiver for introductory care and supervisory visits Assist HR with office administration tasks such as organizing personnel files, performing background checks and preparing paperwork.
Job Requirements Required: One (1) year experience with Intake or Care Coordination, One (1) year HR Assistance experience (preferred)
