REMOTE : FULL TIME PERMANENT POSITION LPN's:
The (LPN) Care Navigator 1 is responsible for performing Comprehensive Medication Reviews (CMR) for
members enrolled in the Medication Therapy Management (MTM) program. The nurse provides telephonic
outreach focused on helping members navigate complex medication regimens, patient safety and
adherence concerns during the CMR. This position has no direct reports.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Examine clinical programs information to identify members for specific clinical interventions
by utilizing established screening criteria
Perform telephonic CMRs with members
Deliver all required information, intervention, and member communication in timely manner; track
and log appropriately
Analyze drug usage reports to identify problems and recommend solutions
Develop strategies to decrease pharmacy costs for members
Promote company’s clinical programs and resources
Facilitate member clinical program engagement
Support quality improvement programs
Other duties as assigned
High school diploma or GED (this will be verified!)
Must maintain an active Licensed Practical Nurse (LPN) license in good standing and without
restrictions in at least one state (License will be verified)
Minimum 2 years’ experience in a clinical setting
Experience in case management or disease management
Strong data analysis and presentation skills

Medicare Part D knowledge
Telephonic case management experience
Bilingual in English and Spanish
Includes a typical office environment, with minimal exposure to excessive noise or adverse environmental
Ability to sit for prolonged periods of time.
