LPN- Home Health PRN

Envision Healthcare at Home (formerly Bridges)  is currently seeking a dynamic Full time LPN for Home Health to provide exceptional care to patients who require one-to-one attention and monitoring in their home in the Denver, CO area.  
  
Responsibilities include:

• client assessment
• fulfilling patient care plans
• intervention and overall care of patient care 
• following physician orders and act in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.

Minimum qualifications

• Graduate of an accredited or state approved school of practical nursing
• Licensure as a LPN in the State of Colorado
• A minimum of one year of experience in an acute care setting; home care experience preferred.
• Phlebotomy and wound care experience preferred.

Pay rate:  $50/visit and hourly rate of $32hr for admin time (meetings, trainings etc)
