Position: Case Manager, Foster Care Prevention, (Full-time, Salaried) Reports To: Executive Director
Start Date: February 1, 2018

Case Manager Job Summary: Ēma South Florida seeks a responsible Case Manager to oversee client needs and coordinate with community partners and agencies. We are looking for a highly relational, organized, and driven professional who believes in facilitating personal growth and family preservation for every client. The Case Manager is responsible for coordinating referrals and case plans for young mothers at-risk of child abuse and neglect. This position will oversee weekly Ema Group Programs, monthly reports, and NPP Assessments. The Case Manager will also assist in organizing, developing, and integrating stable program operation and growth. She/he is to lead and manage referrals and clients so they are successful in their goals and responsibilities.

Key Responsibilities:

1) Coordinate and oversee all incoming referrals, completing intakes, and determining action plan

2) Advocate for needed services with local partners and agencies

3) Monitor client case plans by verifying attendance, observing and evaluating clients, obtaining additional resources, intervening in crises, and providing personal support

4) Promptly deliver monthly reports pertaining to clients to referring agencies

5) Assure every client receives consistent and attentive care with the Ema Group program and outside community referrals

6) Effectively communicate, both written and verbally to staff, clients, and partners

7) Address client concerns with appropriate protocol

8) Facilitate Ema Groups (1-2) and assist in organizing food/materials for all meetings

9) Cultivate a team culture by working directly with Ēma Group facilitators, mentors, and volunteers to assure program stability and quality assurance

10) Assist in outreach events, promotional tables, speaking engagements, and interviews

Qualifications Required of This Position:

· Track record of spiritual maturity and servant leadership

· Strategic leader who executes

· Associates degree in related field and/or two years equivalent experience

· A passion to empower vulnerable moms and preserve families

· Experience in cultivating and nurturing strategic relationships

· Verbal and written communication skills

· Ability to work in a fast paced, flexible environment

· People oriented

· Organization, time management, and problem-solving skills

· Ability to prioritize projects and meet deliverables and deadlines

· Proficient in Microsoft Excel, Word, and PowerPoint
