JOB SUMMARY
The Social Worker promotes social work intervention and consultation upon request/referral from patients, families, health care professionals, and other community members. Services include assessment, information and referral and direct social work services.
As part of the Quality and Resource Management Department, a goal of these services is to enhance patient/family awareness of disease prevention/health maintenance, as well as promoting wellness.
QUALIFICATIONS
- Masters of Social Work degree from an accredited School of Social Work.
- A minimum of two years experience in a health care setting.
