About you:
The ConcertoCare Psychiatric Nurse Practitioner supports the patient, behavioral health clinicians, and primary care teams as part of an integrated practice through regular consultation on an assigned caseload of patients. This is an ideal position for a Psychiatric Nurse Practitioner who seeks an opportunity to contribute to the health and wellbeing being of highly complex patients, enjoys a collaborative multidisciplinary team-based approach to care, and is excited to engage in developing and nurturing our innovative, value-based clinical model focused on caring for patients with complex and social support needs that the current health system does not serve well.
You will:
- Advise the Primary Care Provider and behavioral health clinicians on appropriate diagnostics, medications, and other treatment strategies, also providing the care team with educational support on diagnosing and managing behavioral health conditions in older adults and adults with serious illness.
- Provides clinical services virtually and, where feasible and clinically indicated, at clinic sites and/or in patients residences, under the supervision of the Behavioral Health Medical Director.
- Demonstrate a spirit of collaboration, team-work, and creative problem-solving.
- Evaluate and treat mental health problems, including diagnosis, investigation, treatment, and referral as appropriate.
- Provide psychiatric consultation to members of primary care team, including urgent consults for complex patients or acute situations requiring psychiatric input.
- Collaborate with a growing high-quality care team build integrated treatment approaches for individual patients, identify additional specialists based on patient needs, capture learnings from specialists that improve overall care.
- Support clinical supervision needs of behavioral health clinicians.
- Contribute to detailed assessments of patient needs and detailed care plans to meet those needs through multi-disciplinary team engagement.
- Collaborate and coordinate with staff in the following areas for ConcertoCare patients: ED personnel, specialty mental health clinics, inpatient care, rehabilitation care, skilled nursing, assisted living, home care, and skilled home care organizations.
- Identify opportunities for practice improvements, pilot test new approaches, institutionalize proven practices across the organization.
- Create a culture of learning and teaching among the care team and with patients provide in-service training to staff, identify patient education needs, and coordinate outreach programs as appropriate.
- Support clinical staff on matters related to patient engagement, motivational interviewing, and health behavior change.
- Analyze data at the patient level and the population level to identify trends and develop interventions to produce better outcomes and mitigate clinical risk.
- Maintain high quality, fully up to date medical records

Requirements
- Masters Degree in Nursing from an accredited National League of Nursing graduate school
- Board certification in psychiatric/mental health nursing (PMHNP/ PMHCNS); A current, active, unrestricted licensure as a Washington NP with prescriptive authority, or willingness to obtain. Willingness to become licensed in other geographies is required.
- Prior clinical experience (> 2 years) as a Psychiatric Nurse Practitioner, preferably in the areas of geriatric mental health and/or primary care mental health and/or consultation-liaison psychiatry and/or collaborative care. Prior experience treating individuals with severe mental illness and co-occurring substance use disorders highly desirable.
- Demonstrated ability to collaborate effectively in a team setting

- Ability to quickly synthesize medical and psychiatric data and formulate effective and evidence-based clinical recommendations
- Skilled at using online tools and technology to deliver care and communicate with patients and colleagues
- Knowledge of Medicare and Medicaid care management requirements
- Strong clinical skills and ability to implement evidenced-based care.
- Ability to manage patient complexity and multiple clients with diverse needs
- Demonstrate the ability to triage and apply critical thinking skills.
- Ability to communicate effectively in writing and verbally.
- Demonstrate ability to perform multiple concurrent tasks with minimal supervision and meet deadlines.
- Ability to work in a fast paced, dynamic environment and work well with others on a team.
- Knowledge of Medicare and Medicaid populations a plus.
- Proficient in computer skills to include Microsoft Office Suite (Outlook, Excel, PowerPoint, Word).
- Knowledge and ability to navigate internet-based tools and applications.
- Demonstrates high level of professionalism.
