Job Description

JOB SUMMARY

In accordance with agency Mission, Vision, Values, and Goals, the QMHP (Spring House) provides assessment and treatment planning for residents admitted to the program, and insures all written documentation is clinically appropriate, correct, and meets agency standards.  Interacts daily with team affairs, attends meetings, provides 1:1 and group services, and consults with the supervisor and CSS to provide quality care to the clients within the Residential Services program.

PRIMARY FUNCTIONS

 

Completes assessments on all admissions into Spring House according to departmental guidelines, using Spring House checklists, on the day of admission to include, but not limited to: assessments, DLA-20, individual treatment plans, Adult Status Reports, Diagnostic Review forms, Face Sheets. Must meet CMHS productivity standards.

 

Enters all required documentation (Status Reports, DLA’s, etc) into CIMOR in a timely fashion. Utilizes other health information resources (Care Manager) to gather needed health information on admissions and consumers receiving treatment at Spring House.   

 

Schedules and monitors fire, tornado, and other required safety drills. Attends Fire and Saftey Committee meetings quarterly, or as scheduled.  

 

Manages the census by completing daily census update and adding and discharging assignments on day of admit/day of discharge. Maintains the accepted/denied spreadsheet for data and performance measures.

 

Collaborates with Adult Community Support Department in managing discharges of new CPR clients, ensuring they have an assigned CSS and follow up appointment prior to discharge. Ensures that the assessments, treatment plans and discharge paperwork for non-CMHS clients is sent by medical records to the treating agency.

 

Assists in the clinical treatment and milieu of the program by interacting with consumers on a one to one and/or group basis when appropriate as dictated by client need or as assigned by supervisor and/or coordinator. Provides back-up to CSS in daily group to ensure daily billing for each client.

 

Enters in the weekend group activity into the EMR according to the group documentation form completed by the weekend staff.

                                                                                                                                 

Participates in and follows all agency and department policies/procedures regarding treatment and documentation practices, adherence to DBH and Medicaid standards, QA/QI procedures, and emergency procedures.

 

Provides back up screening coverage when supervisor and CSS are unavailable. Completes billing documentation when conducting screening.

 

Follows all agency policies and procedures including emergency and corporate compliance policies/procedures 100% of the time.

 

SECONDARY FUNCTIONS

 

Assists with house duties as assigned by supervisor when needs arise.

Participates in agency committees as assigned/interested.

All other duties as assigned.

QUALIFICATIONS

Education:

Master's degree in psychology, social work, counseling or closely related human services field.

Experience:

Must have at least 2 years of experience working with the assigned population (prefer 3-4 years).

License, Certification or Registration:

Not required

Special Skills:

Thorough knowledge of current DSM

Proven written/verbal communication skills (including accurate report writing)

REQUIREMENTS FOR POSITION

Must become Privileged staff according to Medicaid guidelines to provide services to assigned population. Must acquire and maintain training related to job performance as outlined by DMH and Medicaid guidelines. Must complete 16 hours per year with documented attendance of professional development/training or education related to case management, community support and to the special needs of serving adults with serious and persistent mental illness.

DEGREE OF SUPERVISION

Receives direct supervision from the Spring House Supervisor and indirect supervision from Coordinator of Residential Services.  Treatment plans and assessments will require the physician's review and signature.
