RN - Registered Nurse - Care Manager

Compass Healthcare Consulting & Placement is conducting a search for a Registered Nurse, RN for a Home Care, Care Manager position with a Healthcare Organization in New York. This is a field position with visits available in Manhattan, Bronx, Brooklyn or Queens. Qualified candidates will have NY RN license; will have a minimum of 1 year of Home Health, Acute Care or Medical-Surgical nursing experience - CHHA/LTHHCP experience preferred; will have knowledge of OASIS and will have knowledge of Microsoft office, Sign on bonus of $15,000!

Registered Nurse - Care Manager - RN CHHA

The RN Care Manager will be responsible for coordinating, implementing and continually evaluating the home care needs of patients.

ESSENTIAL JOB DUTIES

• Create the initial comprehensive and interdisciplinary patient centered home health plan of care; update the home health plan of care in collaboration with the physician.
• Perform a comprehensive physical, psycho-social and safety assessment via examination and observation
• Effectively case manage the care and services for assigned patients resulting in positive clinical outcomes and minimized risks of re-hospitalization.
• Support the company's financial goals, including accurately completing all OASIS data sets to ensure the correct reimbursement is received.

QUALIFICATIONS

• Current NY RN License
• Associate's degree in Nursing, Bachelor's highly preferred
• Minimum of 1 year of acute, Medical-Surgical nursing experience; CHHA/LTHHCP experience preferred, will train those with recent Acute Care or Med/Surg experience!
• Driver's license and reliable car to make home care visits
