How will a Therapist be successful in this role?
Under direct supervision of the ACT Team Manager; provides clinical and support services to consumers and/or their support systems utilizing the evidence based Assertive Community Treatment Model. Candidate is well versed in providing evidence-based treatment modalities including but not limited to Integrated Dual Diagnosis Treatment, Illness management and Recovery as well as Individual Placement and Support. May operate in a residential environment, community based, office based, or in an outreach environment, such as in homes, hospitals, crisis units and/or outpatient clinic. Candidate will serve those individuals presenting with the highest level of clinical intensity at the agency, and primarily those individuals with a serious and persistent mental illness.
Job Duties and Responsibilities:
- Provides counseling, therapy and/or psychotherapy to consumers and families as appropriate; prepares treatment plans, discharge plans, and follow-up care programs, provides therapeutic crisis intervention and emergency services as required, with special emphasis on serving consumers who have a primary severe mental health disorder as well as a secondary diagnosis of substance abuse/dependence.
- Complete treatment planning and care coordination in accordance with Colorado 27-65 rules and regulations.
- Completes ASI/ASAM to determine appropriate level of care for consumers and provides appropriate referral and coordination with outside substance abuse treatment facilities.
- Provides care to consumers in a manner that recognizes the human potential, is empathic and demonstrates unconditional positive regard while promoting safety, comfort and the therapeutic environment.
- Maintains confidentiality of consumer records, particularly in regard to 42 CFR Part 2 substance abuse regulations.
- Provides case management services to consumers as needed.
- Facilitates and contributes to the accomplishment of tasks by consumers and/or families; empowers consumers to reach optimum level of achievement; sets positive example and provides role modeling as well as technical expertise to accomplish goals.
- Documents intervention and program activity according to Community Reach Center policies and procedures.
- Attends and participates in group sessions, in-service education and training opportunities, meetings, supervisory sessions and special events.
- Participates in the review of clinical issues and program policies and procedures.
- Maintains accurate and timely clinical records consistent with Center’s standards (progress notes are entered within one day or providing the service).
- Consults with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapy and/or casework, refers clients to appropriate service agencies as required.
- Consults and coordinates CMHI-FL and CMHI-Pueblo as needed for all consumers that require hospitalization.
- Consults and coordinates with Adams County Sherriff’s Office, Adams County Public Defender’s Office, Adams County Family Court, 17th Judicial District Probation Office, District Attorney-17th Judicial District and other community providers as appropriate for the identification and treatment of individuals in the criminal justice system in need of mental health intervention.
- Provides billable service each year according to Community Reach standards.
- May provide parenting classes and/or other psycho-educational programs.
- Participates in and utilizes opportunities that provide professional growth through attendance at center in-services and other conferences and by conducting consultation and education functions as indicated.
- Projects a positive image to other agencies and general public.
- Assists in pinpointing program and management problems and participates in problem resolution.
- Participates in on-going program development, implementation and evaluation.
- Promotes cooperative working relationships and positive team atmosphere.
- Unlicensed clinical staff will receive supervision as appropriate to their position. It is the responsibility of the supervisee to make substantial efforts towards obtaining licensure and both supervisor and supervisee must be actively engaged in the supervisory relationship. The number of clinical supervision hours provided to unlicensed staff may vary upon the supervisee’s FTE status, job description, and other factors.
Education / Experience:
- Minimum of a Master’s Degree in a counseling related field or a Doctorate degree in Psychology from an accredited university.
- Minimum of two years direct clinical experience.
- Minimum of at least one year of direct clinical experience working with individuals diagnosed with a Serious and Persistent Mental Illness.
- Communication, organization, time management, interpersonal, and evidenced informed clinical skills.
- Knowledge and understanding of clinical social work and mental health counseling principles, methods, procedures, and standards.
- Familiarity with DSM IV and diagnostic techniques.
- Knowledge of ASI/ASAM level of care assessments.
- Professional counseling licensure preferred (LP, LPC, LMFT, LCSW).
- CAC II or above is required-will substitute LAC or similar substance abuse license, or experience and training, equivalent to CAC/LAC certification/licensure from another state with agreement to pursue CAC II+ credentialing.
- Must have a valid drivers’ license and maintain a good driving record while employed with Community Reach Center’s programs.
- Must have QMAP.
