- Monitors and supervises the new hire’s orientation and work processes; case management skills; competence with computer software’s; HOW/member interactions; clinical competencies, communication with other healthcare providers and within IDT; documentation standards. - Carries a modified panel of patients and assists with managing panels vacated by HOW’s leaving the site or organization. Preceptor utilizes panels to train new HOW’s and transfers the panel to the new HOW upon successful completion of the orientation phase (first 60-days). - Develops and coordinates educational experiences and clinical training for novice and newly hired staff on an individual and group basis and monitors progress of CCA Clinical Orientation Competency Program. - Develops individual training plans based on clinician’s needs. - Provides leadership with respect to identification and dissemination of culturally and linguistically appropriate health education; supports HOW’s in safely practicing to the full extent of their education and training. - Participates in and uses evidence-based research to identify strategies for improving professional development activities, and patient centered outcomes. - Ensures educational activities are congruent with CCA’s mission, goals, values, priorities, and resources. - Assists adult learners to identify both their learning needs and the most effective activities to meet those needs. - Provides an appropriate climate for learning and facilitates the adult learning process by developing, planning, and presenting educational activities and directly/indirectly fostering the development of competence in the HOW staff. - Completes 90-day performance evaluations of new HOW’s and participates in performance management activities as necessary (with site leadership and HR support) - Upholds ethical principles in all aspects of practice. - Models behaviors that reflect affiliations with professional organizations as well as the community.
