Job Description

JOB TITLE: Dietitian

REPORTS TO: Rehab Director

STATUS: Exempt

 

The following position description contains representative examples of work that will be performed in positions allocated to this classification. GENERAL PURPOSE:

Plans, implements, supervises and educates the intake of specialized diets with appropriate patients or groups.

 

QUALIFICATIONS:

· Must be a registered Dietitian or Nutritionist

· Must have at least one (1) year experience in nutritional care of the elderly.

· Holds a valid and current license in the state employed.

 

· Must be able to relate positively and favorably to patients, employees, families, and outside agencies and to work cooperatively.

· Must be capable of maintaining regularly scheduled attendance.

· Must be proficient in the use of computers, remote video, phone, fax, MS Office Suite.

· Demonstrates good verbal and written communication, and organization skills.

· Possesses and maintains current CPR certification.

· Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

· Must be capable of performing the job functions of this position with or without accommodations.

 

ESSENTIAL JOB FUNCTIONS:

- Develops and implements nutritional interventions based on the assessment of nutritional needs and the plan of care.

- Evaluate and report results of nutrition program appropriately.

- Instructs and works with patients or home care personnel to:

- Provide appropriate counseling to individuals, families/caregivers or staff in the home or group setting.

- Evaluate and instruct on nutritional principles, dietary plans, food selection and economics. Provides coaching and suggests appropriate dietary alternatives based on health assessment, activity levels, nutritional goals or habits.

- Adapt plans to individual’s life style, as indicated.

- Reevaluate nutritional care by providing appropriate follow-up.

- Communicate appropriate dietary history and nutritional care data in the clinical record.

- Records subjective and objective information, assessment, and treatment plan, in the clinical notes as appropriate.

- Communicates with clinical team regarding patient’s progress and treatment goals and/or makes appropriate presentations on nutrition and overall health benefits.

Patient Rights Function:

- Maintain patient confidentiality.

- Understand and comply with patient rights regulations.

 

Regulatory Compliance Function:

· Maintain a working knowledge of government regulations and ensure compliance.

 

Other Functions:

· Adheres to all of the Company’s policy and procedures.

· Performs other duties as assigned.

Physical and Sensory Requirements (With or Without the Aid of Mechanical Devices):

Walking, reaching, climbing, bending, lifting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smell and temperatures, ability to read and write, ability to communicate with personnel, patients, and outside agencies. Ability to present to groups and the ability to remain calm under stress.
