JOB TITLE: ICM Supervisor PROGRAM: Intensive Case Management FLSA STATUS: Exempt JOB SUMMARY: Supervises, guides, and/or instructs the work assignments of staff and oversees the clinical activities of the Intensive Case Management (ICM) Team. Performs psychosocial assessments, treatment planning/monitoring and discharge planning. Provides counseling/therapy to individuals with SPMI and requiring complex interventions.
Performs commitment determination and/or executes order of involuntary commitments (1013). May also provide clinical supervision towards licensure if eligible and as needed DUTIES AND RESPONSIBLIES: Supervising and Management of ICM team staff 30% Assessments and treatment planning 20% Participates in Treatment team meetings and reviews 10% Maintaining accurate treatment records, spreadsheet & reports 30% Completes trainings 10% Direct care-1013'; Assessments, Counseling, Completes discharge summaries and Level of Care determinations, Treatment planning Administrative... Provides staff supervision, reviews and addresses weekly error, productivity and authorization reports, timely completion of agency trainings, completes all reports and data collections as needed, follows daily schedule as directed.
Leads and participates in treatment team meetings Maintain correspondence with DBHDD and hospitals regarding ICM clients Maintains licensure/credentials by completing all recommended CEUS per licensing board requirements KNOWLEDGE: Knowledge of HIPPA and compliance standards Assessment tools (ANSA/CANS, CSSRS, ASI) Understanding of recovery principals Knowledge of evidence based practices within service provision DSM-V SKILLS: Program Management Providing Leadership Analyzing information/data and making decisions Crisis intervention management Diagnostics Person centered planning ABILITIES: Collaborative documentation Ability to work within DBHDD guidelines Supervising and evaluating employees Establishing and maintaining professional boundaries Meet agency productivity standards while maintaining a high quality of therapeutic care KEY PERFORMANCE INDICATORS (KPIs): Maintain Agency performance expectations 100% of the time. Complete all required documentation same day services are delivered/end of shift (except for after-hours notes which documentation is due by 11:00 AM next business day Complete and provide quality, collaborative service documentation 100% of the time. 99% accurate on entering information in EMR.
100% of require trainings will be completed within specified time frames. PHYSICAL REQUIREMENTS AND WORKING ENVIRONMENT Answer phones and sit for prolonged periods of time. Standing and bending to work with network printers and inventory of supplies.
Lift up to 25 pounds when moving office supplies. Typing and manipulating the computer screen and keyboard. Tolerate periods of time in a loud environment.
MINIMUM QUALIFICATIONS: Education - Master's Degree in Social Work, Counseling or related field Licensure/Certification - LAPC. LMSW, LAMFT LPC, LCSW, LMFT or license eligible. Experience - Minimum three (3) years' experience in human services field Other - Criminal background check clearance, official academic transcripts, valid driver's license/ID, drug screen, TB test, eligible 7yr MVR.
PREFERRED QUALIFICATIONS: Licensure/Certification - LAPC. LMSW,LAMFT LPC, LCSW, LMFT Experience - Three (3) years Supervisory experience (SPMI, Addiction, Children, etc
