Job Description
At Clinical Associates , we make being healthy easier. We connect our patients to a seamless system of integrated medical care, which includes in-house specialists in 10 areas of practice, from cardiology to podiatry. We also offer unique services like our ambulatory surgery center and nuclear stress testing facility. Our more than 50 physicians, specialists and credentialed health care providers are assisted by 175 trained support staff. We do our best to take care of everything for our patients – records, scheduling appointments, coordinating with providers and specialists, and each of our offices has laboratory centers for tests and blood work on site. The Population Health Clinical Care Coordinator will work with the Practice Transformation Specialist and Practice Site Manager to support the formulation and execution of the CMS program Initiatives, strategies, and functions, including supporting development and execution of any and/or all phases and functions of the program. WHAT YOU WILL DO: Provides an experience of efficient yet nurturing environment for patients and families i.e., greets patients and visitors via phone and in person in a pleasant manner, making eye contact when the encounter is in person. Assists with monitoring practice and provider level patient attribution (empanelment) and compilation of data for monthly, quarterly reporting. Maintains patient panel listings by scheduling necessary appointments, follow ups and performs the necessary documentation according to the needs of the Population Health program. Tracks test results, notifies patients, and provides patients with pre/post-op instructions as directed by the practitioner. Possible Travel Other possible assigned duties: Performs the necessary functions to meet the Care Transformation requirements of the Population Health program including working on the daily Transitional Care Management (TCM) reports. Support the Population Health Department to meet quality metrics for "Gaps in Care" initiatives. Assist with patient population outreach phone calls to close "Gaps in care" for CA's participating incentive programs. Schedules patient labs/tests, necessary visits, hospital follow ups, ER follow ups, and referrals to other practitioners as requested and completes all related documentation. Assist with marketing campaigns - Vaccine, AWVs WHAT YOU WILL BRING: High School diploma or equivalent. Excellent Customer Service Skills. 3 Years of Experience as a Medical Assistant in Primary Care NextGen Experience a plus Medical terminology. Knowledge of anatomy and chronic conditions is REQUIRED. Basic arithmetic and money calculating skills.
