Behavioral Health Specialist

The Behavioral Health Specialist (BHS) promotes, provides and
coordinates high-quality behavioral healthcare in the Primary Care
Department. The BHS supports Primary Care Providers (PCPs) to develop
evidence-based treatment plans that address the wide range of mental
health and substance use disorders presented by patients in Primary
Care. Activities include integrated assessment/treatment visits in
collaboration with the PCP, and, follow-up on Behavioral Health (BH)
visit aftercare referrals. The BHS actively facilitates external
referrals, assesses and diagnoses mental health disorders requiring
referral to the consulting psychiatric nurse practitioner, and consults
with medical providers to develop multi-factorial treatment plans. The
BHS develops and maintains a network of outpatient mental health and
substance use disorder providers in the community and collaborates with
them to insure Primary Care patients locate needed treatment. Master's
Degree in Social Work (LICSW), or Clinical Mental Health Counseling
(LCMHC). At least 3 years' experience in a clinical Behavioral Health
setting with responsibilities including diagnostic assessment, treatment
planning. Risk assessment and intervention. Must have demonstrated
experience assessing and treating people with substance use disorders,
serious and persistent mental illness, and treatment-skeptical
individuals. Knowledge of levels of care. Demonstrated knowledge or
training in one or more evidenced based practices including: Cognitive
Behavioral Therapy, Dialectical Behavioral Therapy, Motivational
Interviewing. Engages in continuous professional development related to
the role of integrated BHS. State of New Hampshire independently
licensed clinical professional: LICSW or LCMHC. Licensed Alcohol and
Drug Counselor (LADC) in addition to clinical license is desirable.
Familiarity with mental health and SUD service delivery systems in
Monadnock/Eastern Vermont regions helpful. Awareness of models and goals
of Behavioral Health/Primary Care integration.
