POSITION TITLE: Community Life Program Coordinator

SUPERVISION BY: Community Life Program Manger

DAYS/HOURS OF JOB: Monday - Friday 9:00 am - 3:00 pm

GENERAL DESCRIPTION OF POSITION:

Assist the Community Life Program Manager with daily programming offered during the Community Life Program. The Community Life Program Coordinator will provide a structured day program for REACH, SAIL and community members. The Community Life Program Coordinator will provide meaningful daily activity to the Chapel Haven Schleifer Center clients and residents.

ESSENTIAL RESPONSIBILITIES OF THE POSITION

  • Facilitate daily activities planned by the Community Life Program Manager.
  • Instruct and assist clients with daily living skills. (Preparing lunch, budgeting, hygiene, healthy relationships, etc.).
  • Follow directions provided by the Community Life Program Manager.
  • Maintain appropriate documentation (attendance, incident reports, progress notes via SetWorks, emails, etc.)
  • Transport clients to and from any day trip, event, or volunteer site offered by the Community Life program, following Chapel Haven's van usage protocol. Use of own vehicle may be required.
  • Take attendance of group, handle petty cash following protocol, carry materials needed for the trip i.e., tickets, epi-pens, meds & money, meet client needs & support the clients while in the community
  • De-escalates arguments and other potentially stressful situations through adept communication.
  • Staying calm and responding quickly to unexpected situations.
  • Attend agency, program, and staff development meetings as required.
  • Modifying a planned course of action in response to unexpected delays or new circumstances.
  • Maintain the safety and well-being of all Chapel Haven Schleifer Center clients.
  • Obtain a Public Services License (PSL) within 90 days of the position start date.

MINIMUM EDUCATION REQUIREMENTS

Bachelor's Degree preferred

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
  • Minimum one-year experience with the developmentally disabled population in a recreational and/ or instructional setting.
  • Cultural sensitivity and proven ability to work with diverse populations.
  • Knowledgeable computer skills and ability to use Microsoft Programs (Word, Excel, Publisher).
  • Highly Creative and organized
  • Valid CT driver's license and Public Service License.
  • CPR, Frist Aid Certifications, State of CT Medication Certifications

PHYSICAL REQUIREMENTS
  • Ability to stand, sit, walk, and climb stairs for periods of time
  • Ability to left 20 lbs. (24-pack of bottled water, grocery bag with groceries)
  • Ability to drive 12 passenger Van safely approved by the Community Life Program Manger

$17.25 per hour
