Clinical Liaison (RN LPN)

We are currently interviewing for two Admissions Coordinators for our centralized admissions hub located in Muskego, WI.  These positions are in-office positions M-F with some phone/admissions on-call responsibilities.  We are seeking a Registered Nurse or Licensed Practical Nurse for these positions.  Previous experience in skilled nursing facility in admissions is highly preferred.

Our inpatient clinic is looking for an experienced Registered Nurse RN or Licensed Practical Nurse LPN to serve as a Clinical Liaison. In this role, you respond to patient referrals by conducting a pre-screening of their medical records and insurance information to determine whether they are eligible for admission. If the requirements for admission are met, you may assist them with scheduling their initial appointment or treatment. You also help coordinate discharges and transfers. Although we do have strict requirements for admission, one of your main goals is to keep our clinic as close to capacity as possible, so you also promote our healthcare services to qualified patients and their families.

Duties and Responsibilities

• Respond to patient referrals
• Conduct pre-screening of potential patients to determine their eligibility for admission
• Review medical records
• Process payer information and ensure requirements are met
• Coordinate patient discharges and transfers
• Promote our services to qualified patients and their family members or caregivers

Requirements and Qualifications

• Completion of an associate or bachelor's degree program in nursing
• Current RN or LPN state license
• Clinical or case management experience
• Analytical skills
