Job Description


The Adult Foster Care (AFC) Coordinator is assigned to complete the AFC Care Manager job and assist the Fiscal department with processing Caregivers’ payments. The coordinator collects, reviews, and ensures the documents required to participate in the programs as a consumer and/or caretaker are completed as outlined by the regulations.


  • As a member of the Interdisciplinary team, participates in Initial and Ongoing Assessments of the health and functional status of enrollees and eligibility for the AFC program.
  • In collaboration with the AFC RN, consumer and family, and Primary care team (if present), ensure that the community-based care plans and related service packages are maintained to improve or maintain an enrollee’s health and functioning.
  • Process and submit bi-weekly timesheets to ensure timely caregivers' payment to Fiscal.
  • Keep track of yearly Medical and Non-medical leave of absence and submit monthly billing to Fiscal.
  • Assist in arranging and coordinating provisions of appropriate long-term care and social support services for AFC consumers. These may include but not be limited to ADL and ADL assistance, Home-delivered Meals, and Transportation.
  • Monitor the client’s needs, the appropriate provision, and functional outcomes of the AFC services in an ongoing manner through home visits and telephone.
  • Maintain ongoing communication with consumers and Caregivers.
  • Respond promptly to consumers' and/or Caregivers’ requests.
  • Work in collaboration with external agencies involved in consumer care, such as Home Care (HC), Certified Home Health Aide (CHHA), and Adult Day Health (ADH).
  • Prepare documentation for consumers’ files, agency database, and other Interdisciplinary and/or Primary Care Team members.

  • Report suspected Fraud, Waste, and Abuse (FW&A) as required by CBES FW&A policy.
  • Protect Consumers’ Personal Health Information (PHI) and report any suspected security breaches.
  • Participate in staff meetings, agency committees, outreach activities, and task forces as appropriate.
  • Attend program training sessions to improve skills and knowledge relevant to the role and share knowledge with other staff members.
  • Other duties as assigned as outlined in the attachment if applicable.
  • · Must be able to speak and write fluently in English and Russian.
  • · Bachelor’s Degree in Social Work or Human Services, or two three-plus years of experience in case management, service coordination, outreach, or advocacy.
  • · Ability to assess clients and their needs and knowledge of the community-based service network.
  • · Excellent interpersonal and problem-solving skills are required.
  • · Ability to work with a diverse population.
  • · Access to a car preferred.


  • · Ability to lift and carry objects frequently weighing 10 to 15 pounds.
  • · Ability to climb stairs.
  • · Ability to travel to the community to perform visits at consumers’ homes.
  • · Work requires regularly standing, stooping, and bending.
