Job Description

We are looking for compassionate Case Managers who are ready to make a difference in the lives of women returning to Contra Costa County - Central and East County, either full time or part time

As a Centerforce WEST Case Manager, you will be required to work partly in the community and partly in the office.  A typical work schedule for a full-time Case Manager in Monday-Friday.

Community based Gender-Responsive Case Management includes:

  • Conducting screenings and gender responsive assessments
  • Creating an Individual Action Plan (IAP) in collaboration with participant
  • Regular meetings with program participants to gauge and encourage progress towards goals identified in their IAP
  • Making referrals in and out of the Reentry Data System
  • Contact with Probation or Parole on behalf of the participants
  • Entering case notes into a secure data system.  Implement program protocol, complete tracking documents and certificates of completion
  • Attend trainings and meetings related to the reentry population

Program Administration

  • Complete day to day administration and program operations such as required data base documentation and reports
  • Regular communication with co-workers, manager and reentry network partners

Qualifications

  • An undergraduate degree in Human Services, Social Work, or a Certified Alcohol Drug Counselor certification
  • Three years  of related experience (preferably case management)
  • Ability to work as part of a multidisciplinary team with people from diverse ethnic, cultural, economic, formerly incarcerated and gender identification communities required
  • Able to communicate effectively and tailor communications to a variety of audiences
  • Due to work responsibilities, ability to work within the San Francisco Bay Area County jail systems and the California State Department of Correction and Rehabilitation is required
  • TB and COVID 19 tests and DMV clearance
  • Reliable vehicle for transportation
