JOB SUMMARY: Facilitates telephonic communication with patients transitioning from hospital to home as assigned. The Care Transition nurse will utilize the nursing process and computerized database to facilitate communication with patients transitioning from hospital to home. The nurse is expected to work closely with providers, nurses, and all other members of the organization by demonstrating effective communication. The nurse is accountable for efficient managed care and client satisfaction via follow up telephone calls to ensuring a safe transition to home and prevention of readmissions.
Uses appropriate knowledge and skills in the care & interaction with customers across the age continuum.
Performs care according to hospital / departmental policies & procedures.
Identifies self and role to all customers.
Ensures that all team members are aware of key information essential to patient care.
Seeks & offers assistance as needed.
Utilizes conflict resolution skills
Communicates in a positive professional manner when relating to customers.
Functions independently and demonstrates leadership in decision making, communication and
problem solving technique to resolve clients questions/concerns.
Participates in committees and in meetings for process improvement as assigned
Gathers historical data and reviews and researches medical documents.
Evaluates patient's record including diagnosis, medication history, discharge summary and plan of care.
Evaluates changes in patient condition and intervenes appropriately.
Evaluates patient allergies and medication incompatibilities.
Documents interventions findings and intervention in computerized database.
Identifies potential risk factors and intervenes appropriately.
Through the nursing process, with the aid of a computerized database and other established resources initiates calls to
Patients discharged from the care of the organization.
Through the nursing process, a computerized database and other established resource materials responds to questions from
Patients regarding recovery, confirms medication, ensures follow up appointments and services have been arranged.
Validates the plan of care for completeness and accuracy
Acts in the role of client and family advocate to assist them through health care system if needed.
Reports pertinent information regarding the client to the appropriate party. Escalates concerns.
Demonstrates excellent communication skill.
Maintains a customer-driven professional attitude,
Assumes responsibility for being familiar with all equipment and demonstrated the ability to utilize equipment and resources in a
Cost-effective manner.
Operates the console of telephone lines and associated equipment,
Collects and inputs data and ensures that complete and accurate documentation is recorded in the computerized database for
Each call.
Education: Current Arkansas Licensure as a LPN.
Experience: minimum 1 year acute care experience
Physical Requirements: This job requires, at various times, the following physical activities; climbing, balancing, stopping, kneeling, crouching, reaching, standing, carrying, walking, pushing, pulling, lifting, finger dexterity, grasping, feeling, talking, and hearing, sometimes exerting in excess of 20-100 pounds of force. The worker is required to have close visual acuity to perform daily activities. May be subject to inside environmental conditions, hazards (including electrical currents and exposure to high heat and chemicals), infectious diseases, and may be required to function around prisoners and mental patients.
