Job Description

The Center Manager is responsible for the leadership and management of the Benson Adult Day Health Center (ADHC) and the Hope Haven Alternate Care Site. This position provides the oversight and management of the Day Center Staff and management of the day-to-day operations, participant flow to, from, and inside the day health center. This includes Day Center staff oversight, participant/caregiver relations, and collaboration with other departmental services. Additionally, the Center Manager assists in the coordination of care planning process and participates and assists in facilitating Interdisciplinary Team (IDT) meetings representing the Center Manager role.



Qualifications:

  • A Bachelor's degree in a field such as rehabilitative therapy, nursing, social work, recreational therapy or related health-care field is required.
  • Knowledge of health care delivery systems, regulations, quality assurance and medical and non-medical needs of the elderly is desired.
  • 3-5 years of management experience is required.
  • Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers.
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public.
  • A minimum of two years of experience with a frail elderly population is required.

RESPONSIBILITIES AND DUTIES:


Essential Job Functions:


  • Assist with coordination of IDT care planning and implementation, including preparation for interdisciplinary conference and daily meetings, family conferences and all related follow-up as needed.
  • Provide leadership and assist in the facilitation of the Interdisciplinary Care Team meetings.
  • Hire, supervise, and perform performance appraisals for Day Center Lead, Pastoral Care, Dietician and Day Center Technicians
  • Participate in daily team meetings, communicate relevant issues to the team and ensure all Day Health Center staff are informed and up to date.
  • Assist with participant/caregiver relations and satisfaction, and initiate activities to resolve participant/caregiver complaints.
  • Orient new participants and caregivers to the program and to the day center.
  • Oversee distribution of center meals and snacks.
  • Develop work-flow processes and programming specific to the department to ensure efficiency in meeting participant needs.
    • Ensure department meets all required regulatory requirements including adherence to policies, procedures and documentation.
    • Demonstrate active participation in strategic planning for both the Day Center and the overall PACE program
    • Assist with audits and monitoring of electronic health records for staff compliance with documentation completion, including both content and timeframes
    • Communicate observations and recommends interventions for participant care
    • Communicate consistent and effective messaging from Leadership and PACE GNO Executive team
    • Complete all other duties as assigned

    • Contact with participants and caregivers under variety of circumstances i.e., irrational, hostile, grieving, actively dying.
    • Subject to frequent interruptions and distractions within the work environment.
    • Probability for unexpected crisis situation to occur.
