Cascadia Behavioral Healthcare, one of the largest community behavioral health providers in the state of Oregon, delivers whole health care -- integrated mental health and addiction services, primary care and housing -- to support our communities. Our approach is recovery-oriented and trauma-informed. This is a full time position at 40 hours per week. The work is performed in both the community with time out on the streets, in homeless shelters, community food programs, and in consumers' homes, with the Street Outreach Team's home base being in Downtown Portland. The schedule is Monday - Friday, 9:00am - 5:30pm.

The Bilingual Street Outreach Worker works independently and as a member of a team to provide street outreach and engagement, housing placement, and time limited home based retention supports to individuals and household members impacted by homelessness and mental illness. The Worker must have strong interpersonal skills necessary to develop positive working relationships within the team and assist participants in developing trusting relationships with other individuals and support systems, and assist all participants in accessing program partner services as well as mainstream resources/health programs. The Street Outreach Worker is responsible for conducting outreach services both on the street and with local social service providers with the intent of securing housing and other essential services to resolve homelessness, while supporting a Trauma Informed Care environment. With the high prevalence of trauma among individuals receiving mental health services, it is required the Street Outreach Worker understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. Services will be primarily provided on the street, in the community and in participant's homes. The primary goal is to quickly place participants into housing and achieve long term housing stability. All services must be able to be effectively provided in Spanish and English, written and verbal communication.

Essential Responsibilities:

This position description is not intended to be an all-inclusive list of responsibilities, skills, or working conditions associated with the position. Management reserves the right to modify, add or remove duties as necessary.

* Conduct outreach and engagement to mentally ill homeless people sleeping outside or at homeless service programs within the City of Portland and Multnomah County in coordination with partner staff at NARA, Urban League and JOIN and our Police Liaisons. Provide focused outreach and engagement efforts to the Latino and Spanish speaking population.

* Provide engagement services to clients utilizing strength based case management, motivational interviewing and harm reduction concepts.

* Timely communication with supervisor, manager and office coordinator on all individuals/households who are receiving outreach supports, or other services. Full and prompt communication and approval for all client households on caseload, and service planning, and potential need for program client/rent assistance.

* Keep informed on each client's individual health and service needs. Includes regular review and feedback on their Cascadia Behavioral Health Assessments and work collaboratively with their Primary Clinician to develop Treatment Plans (ISSPs) with participant. Track and support completion of all tasks related to enrollment as a Cascadia client for clinical and insurance billing purposes.

* Gather all client level data in a timely and accurate manner as required for HMIS/Service Point and contacted funders. This includes entry, exit, and services provided each month.

* Timely completion of all applications related to housing placement and service access. This includes providing quality information to each household engaged on service and housing options, as well as completion and submission of forms, applications and other documentation in collaboration with participating clients. Includes, but is not limited to supportive housing application packets, apartment applications, rental subsidy applications, vulnerability assessments, referrals into BEST and other entitlement assistance programs, food stamps, AMHI programs, Crisis/Respite services and all other programs and resources of value to the participant household.

* Provide translation service to participants as needed with property managers, health care providers and other key or needed supports.

* Provide limited individual therapy and skills training services, including practical support and advocacy to clients throughout the working relationship with focused intent to improve each client's ability to develop informal and formal support systems, with the intent of long-term housing stability, and full engagement in outpatient health and mental health supports.

* Provide in-reach/liaison to community partners serving homeless people under the direction and in collaboration with the team's management and direct services staff and partners.

* Access available housing and community resources and address the barriers that may impede access to housing. Share housing resource information and strategies with supervisor and the team to increase team's effectiveness.

* Provide advocacy services for clients including securing benefits, mental health services, housing programs, medical services and emergency services.

* Complete any documents required for client care, and services provision. Includes coordination with their primary clinician for Multnomah Treatment Fund and internal agency transfer paperwork, Wellness Center referrals, and other appropriate mental health and/or addiction services based on outcome of assessment and consumers' interest.

* Assist and verify all participants have a primary health care provider, and provide service coordination with other professionals and medical providers if a client is hospitalized to ensure timely and appropriate discharge into care. Provide timely follow-up.

* Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations. Maintain timely, professional and accurate and up-to-date documentation as required, associated with billing, assessment, treatment, client's progress and other activities.

* Respect and honor client rights and responsibilities and demonstrates professional boundaries and ethics.

* Assist in crisis situations as appropriate, following emergency protocols/procedures and coordinating with the treatment team with interventions which support a trauma informed environment for clients and staff and maximize safety of self and others.

* Demonstrate responsibility for safety of client's staff and property; is familiar with fire regulations and evacuation procedures at the Royal Palm.

* Adhere to mandatory abuse reporting laws and HIPAA requirements.

* Actively coordinate with partner staff with NARA and Urban League (and others) to quickly engage Native American and African American households with these culturally specific services Link Spanish speaking households with our bilingual team member, and arrange for translation services as needed/eligible. Remain aware of discrimination issues, within the program and in the community, and communicate promptly with management staff around impacts and advocacy/education needs. Work as a member of a team to create a welcoming supportive work environment, and clinical program which effectively serves and supports a diverse client population.

* Liaison role with NRT officers, fostering effective working relationships, demonstrating prompt and appropriate responses, and adhering to agency and HIPAA protocols.

* Complete RFP's for client assistance, and follow all protocols and guidelines consistently and within required timelines.

* Remain alert to safety risks and maintain appropriate professional boundaries within this unique and often challenging work environment.

* Appropriate use of program electronic systems and tools at all times. Includes use of assigned agency cell phone, laptops/netbooks/desktop computers and other office equipment and vehicles.

* Outreach to potential housing providers, with specialized attention to creating partnerships with housing providers who can partner effectively with the participant population.

* Create and maintain a welcoming work and community living environment, model respect, friendliness, team work and compassion.

* Role Specific Responsibilities:

* On-call employees provide relief to regular staff at either or all of the supported housing sites. They require the ability to work a flexible schedule and must commit to working a minimum of 4 shifts a month.

* Staff may be required to work a 12-hour shift, if the need arises, and when coordinated with the program supervisor or manager.

* Complete annual employee training requirements on a timely basis as indicated in the licensing requirements for the facility as well as agency requirements. Attend seminars, training and other educational opportunities in order to develop professional skills and abilities.

* Participate and be on time for all scheduled staff meetings, supervision sessions, clinical supervision sessions, partner meetings and other departmental and agency meetings.

* Perform other duties as assigned

Education:

* The Counselor II must have a Bachelor's degree in a behavioral science field from an accredited college or university or three years of full time professional experience working with adults with severe and persistent mental illness. Demonstrate the competencies to become credentialed as a Qualified Mental Health Associate (QMHA), as outlined in OAR 309-016-0005.

* The Counselor III must have a Master's Degree in a behavioral science field from an accredited college or university. Demonstrate the competencies to become credentialed as a Qualified Mental Health Professional (QMHP), as outlined in OAR's.

* A CADC credential is preferred given the high occurrence of substance use/abuse.

Experience:

Experience providing culturally specific services to the Latino/Hispanic population. Personal or work experience serving this community is preferred.

Specialized Knowledge, Skills and Abilities:

* Conduct a Mental Status assessment and formulate a diagnosis according to the most current DSM (C IIIs)

* Knowledge of Motivational Interviewing, Trauma Informed Care, and principles of Illness Management Recovery, and skills to provide strength based practices.

* Must be able to effectively engage and support Latino/Hispanic and Spanish speaking clients and provide culturally specific services.

* Operate Computer hardware and software at a level to effectively perform the job functions

Other:

* Bilingual proficiency in Spanish is required. Must be able to pass a Spanish language proficiency test.

* Work may include the use of company provided transportation in order to transport clients. Possess a valid driver's license, three-year driving history; clean driving record, ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course. Access and ability to use a reliable vehicle in order to travel to partner sites and community appointments. This does not apply to the Royal Palm and / or overnight positions.

* Must have the ability to perform CPR, First Aid, and properly use AED equipment. CPR/ First Aid certifications are required and must remain current.
