Casa de Salud (Casa) delivers high quality clinical and mental health services for uninsured and underinsured patients, focusing on new immigrants and refugees who encounter barriers to accessing other sources of care. Casa’s vision is to combine low-cost access to treatment with a focus on long-term prevention that results in better health and lower costs. Casa is unique in that it is the only organization in the metro area that offers the new immigrant community low-cost access while also acting as a portal to other services through our collaboration with numerous health and social service organizations throughout the metro St. Louis area.


The Patient Care Coordinator is one of two nurses at Casa. The Patient Care Coordinator uses his or her clinical knowledge to ensure Casa patients achieve timely, high-quality continuity of care both inside and outside Casa’s walls. This includes coordinating patients’ ongoing care within the clinic, providing clinical support to lay case managers, supporting patients in establishing the necessary self-management behaviors needed to manage chronic illness and promote wellness, and supervising the front desk staff to provide coordinated care across Casa’s departments. The Patient Care Coordinator is responsible for ensuring high quality care through responsiveness to patients and integration between Casa and other providers as well as between physical and mental health systems.

The Patient Care Coordinator works in conjunction with the Lead Nurse. The Lead Nurse manages the on-site clinic, making it possible for our 40 plus healthcare professional volunteers to provide medical care to our patient population. In addition to the Lead Nurse, the clinic team includes two full-time Medical Assistants (MAs), four part-time (PRN) MAs, and volunteer providers and interpreters who work together to deliver high quality, HIPAA-compliant, culturally appropriate, and streamlined on-site clinical care for a diverse patient population. The Patient Care Coordinator interfaces with the Lead Nurse to ensure that Casa’s patients and clients receive high-quality, responsive care outside of the clinic in Casa’s other departments.


Coordination of Care Responsibilities

  • Work with departmental coordinators to systematically ensure patient care at Casa is well-coordinated after on-site visits (among the clinic, case management, and mental health departments)
  • Provide consultation on case managers’ cases when clinical knowledge is needed, and facilitate communication with internal and external medical providers
  • Provide telephone triage to patients during business hours
  • Contact patients seen in Casa’s clinic with new or uncontrolled hypertension or Type 2 diabetes at regular intervals to assess their knowledge, motivation, and ability to manage their health, providing additional resources and information when needed
  • In coordination with the Lead Nurse, perform nursing duties in the clinic including occasional blood pressure checks, medication administration, and patient education
  • In conjunction with volunteer coordination staff, orient and support volunteer nurses who provide care to Casa’s patients through the Home Visit Program
  • Audit no-show appointments to ensure proper patient contact/correspondence is completed and personally follow up with patients who are in urgent need of care
  • Review and approve record requests for continuity of care
  • In collaboration with receptionists, schedule patient appointments according to established procedures, including regular review and maintenance of each provider’s patient list

Supervisory Responsibilities

  • Supervise the front desk team, including scheduling and timekeeping, holding monthly meetings with all receptionists, training new receptionists, providing ongoing and annual feedback for receptionists, and ensuring all receptionists understand and are following all procedures
  • Represent the front desk in agency meetings, communicate regularly with the Vice President & COO, and act as the point person within the agency for administrative and reception responsibilities
  • Ensure front desk operations are running smoothly, including maintenance of the appointment calendars, review and follow up on daily front desk reports
  • Monthly reporting to department coordinators regarding front desk numbers and operations
  • Ensure effective coordination with certain outside agencies including interpreters and on-site referral vans
  • Update and maintain the front desk manual
  • Ensure records requests both to and from Casa are processed in a timely manner

Organizational Responsibilities

  • Document in Casa’s electronic medical record in a timely, professional, and accurate manner
  • Attend monthly staff meetings and periodic staff trainings
  • Collaborate with and support other staff and volunteer team members, including medical directors and other nursing staff, as necessary to ensure high-quality, seamless care
  • Protect patients' rights by maintaining confidentiality of all personal and financial information and manage patient records in compliance with HIPAA regulations
  • Promote health literacy in the clinic
  • Other duties as assigned


  • Active Missouri RN license required; BSN preferred
  • Oral and written English fluency required; oral and written Spanish highly preferred
  • Experience in case management preferred
  • Supervisory experience preferred
  • Commitment to working with medically underserved patient populations
  • Strong interpersonal skills and ability to communicate effectively with organizational leadership, Casa medical providers, patients from diverse backgrounds, and colleagues within and outside the organization
  • Strong, collaborative problem solving skills
  • Ability to handle high work volume, prioritize urgent issues, and remain focused on full scope of tasks
  • Good fit with a dynamic nonprofit organization
  • Working knowledge of Microsoft Office Suite and Google Suite
  • Experience with electronic medical record documentation preferred, and ability to learn to use Casa’s electronic medical record required


  • This position is full-time; interest in part-time or PRN availability will be considered
  • Work is performed in a typical clinic and office environment, though must be available to work remotely as needed
  • Occasional evening and weekend activities
  • Average physical effort with some handling of light weights such as supplies or materials on an infrequent basis (10-15 pounds)
