The Intake Department plays a vital role in the day to day operations of Carter Healthcare. The Intake LPN is responsible for assisting with;
•Receives referrals via telephone, FAX, or in person and processes them in accordance agency policy. Collects all data required by policy and procedure at time of referral.
•Assist with referral process and coordinating all necessary disciplines. Notifies staff at locations/departments as requested, contacting staff as necessary. Provides service information to the public.
•Promotes effective customer relations that contribute to agency success.
•Communicates appropriately and is able to understand customer/ patient/ employee needs; satisfactorily answers questions.
•. Data enters patient information into patient databases.
•Performs other duties as assigned or requested.
