CareSTL Health is currently seeking energetic, diligent, and hard-working Registration Clerks to join our team!

The Patient Facilitator establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.


ESSENTIAL FUNCTIONS:

The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned. The Patient Facilitator may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons.


  • Greets and receives all patient/visitors presenting themselves to respective modules for service
  • Answers and direct phone calls
  • Collects all forms accompanying patients; verifies each patient's present address, zip code, and phone number
  • Measures and monitors vital signs
  • Directs patients to nurse and or provider for clarification of treatment and or patient's education
  • Obtains patient signatures on appropriate forms for medical records and billing purposes i.e. release of information, lab
  • Be responsible for internal referrals
  • Sanitizes rooms, sterilizing and autoclaving of special equipments as needed
  • Documents appropriately in EMR
  • Works under the guidance of Supervisor to perform patient care services
  • Addresses patient concerns and queries professionally
  • Attends to patient needs when called
  • Performs basic administrative duties accurately and by deadlines, including printing, copying, and filing patient records
  • Participates as assigned on organization teams

JOB REQUIREMENTS AND QUALIFICATIONS:


Education:

  • High School Diploma or GED required

Experience:

  • Up to 1 year of experience

Knowledge, Skills and Abilities:

  • Must have knowledge of information and techniques in providing excellent patient care
  • Must have knowledge of administrative and office procedures and systems such as electronic transmission via facsimile and/or internet
  • Good computer skills
  • Good self motivational and organizational skills
  • Must have the ability to maintain confidentiality
  • Must have the ability to understand the complexities of integrating services
  • Must have knowledge of data management through use of IT

Transportation Requirement:

  • Must have Reliable Transportation
  • Ability to work all health center sites
