Job Description

The Service Manager will help manage, direct and coordinate caregiver service activities at CareHelpers.

Responsibilities

  • Manage the client experience following an initial handoff call, including client assessment, introduction of caregivers and on-going communication
  • Develop care plan which establishes goals based on specific client needs following initial client assessment
  • Collect constant feedback from family representative involved with client’s care and medical visits in order to adapt care plans, as needed
  • Conduct timely quality assurance calls with clients to confirm the optimal levels of service are being provided
  • Complete initial follow-up and subsequent survey visits
  • Provide care management, as needed, and work with the team to solve escalated client issues or challenges
  • Ensure customer satisfaction measures are developed and met
  • Communicate with clients in reference to cancellations, replacements, schedule changes, etc.
  • Provide ongoing client support in a professional and timely manner and be available to assist with client calls and emergencies, as needed
  • Provide ongoing training and collaborate with the in-field staff to offer support, encouragement and accurately answer questions that may arise about services provided
  • Coordinate and schedule field staff assignments, making certain that all clients are assigned to staff that are qualified to provide the level of care, as determined by the care assessment
  • Work with caregivers to keep them informed of any changes such as care plan changes, adjustments in service hours and any other important matters
  • Communicate directly with field staff on a daily basis and document confirmation and/or changes in scheduling software
  • Respond to time-off requests, canceled shifts, and call-offs by providing backup caregivers and communicating with clients, as needed
  • Ensure system activity log is regularly updated to reflect clients' change in condition, address requests to re-staff, and manage any other caregiver or client concerns
  • Maintain a current patient roster or “casebook” with all pertinent staffing information and monitors schedule changes
  • Coordinate client services for new clients received afterhours
  • Monitor and address caregiver performance
  • Interview, employ, develop and retain qualified field staff to maintain appropriate staffing levels
  • Maintain written coordination notes of communications
  • Respond to both client and caregiver concerns, direct to higher management when necessary
  • Maintain on-call availability after-hours and returns calls promptly and professionally
  • Other duties as assigned

Qualifications and Skills

  • A degree in nursing is required
  • Minimum of five years of experience in nursing, home care or related field.
  • Ability to communicate effectively with customers, both internal and external
  • Ability to function independently, make decisions, problem solve and utilize resources, as necessary
  • Computer literacy is essential with some knowledge of Microsoft Office products
  • Should be a skilled organizer able to manage office files, log books and staff schedules
  • Strong follow-up skills
  • Multiple years' experience working with older adults is preferred, but not required
