In the Care Advocate role, you'll provide telephonic, e-mail and/or community forums to outreach to members in an integrated care management model to reduce modifiable risks factors, encourage program engagement and participation through education, lifestyle management, and referrals to other internal services and resources.The end result is to achieve personal health outcomes by providing support, encouragement, and education to improve well-being and prevent/minimize disease.

Responsibilities & Requirements
+ The Care Advocate would have a High School diploma, with a medical certification such as: Certified Nursing Assistant (CNA) or Medical Assistant and 3 years experience in health plan customer service, Clinical Intake, Provider office, or health coaching or equivalent combination of education and experience
+ Initiates outbound telephone calls or emails to members identified for Care Management Programs for defined care gap initiatives
+ Provide members with support, encouragement and education to improve the member's ability to make informed healthcare decisions, navigate care options and to take actions in order to improve well-being and prevent/minimize disease
+ Strong communication skills with the ability to effectively interact with a wide variety of individuals
+ Track record of customer service excellence
+ Organization skills and proven ability to effectively handle multiple tasks and prioritize workload
+ Knowledge of technology and applications such as Excel, Word and PowerPoint
+ Understanding of care management principles
+ Identify areas in need of improvement in all aspects of program and provide recommendations for solutions
+ Research and problem solve in a creative manner to reach a solution. Ability to identify when it is appropriate to refer to others
+ Ability to flex hours to staff early evening calls
