JOB TITLE: Physical Therapist Adult Program - AquaticsDEPARTMENT: Therapy ServicesSTATUS: Exempt 40 hrs/wkVERIFIED: July 2020REPORTS TO: Director of Therapy ServicesJOB SUMMARY:The primary role of the Physical Therapist is to provide services to clients in the Adult Program. This role will be responsible for the evaluation of the client and the implementation of the treatment plan. The Physical Therapist will also be responsible for appropriate communication (verbal & written) with service coordinators, intake/billing specialists, family members, insurance companies, physicians and Medicaid.
This position is responsible for supervision of a Physical Therapy Assistant. Services provided to clients are both on land and in the pool.DUTIES AND RESPONSIBLITIES:1. Complete thorough evaluation including range of motion/mobility, assess orthopedic needs, movement analysis and functional abilities for the clients that incorporates functional mobility skills following treatment using...
manual therapy and aquatics.2. Educate and demonstrate (verbally and/or in writing) therapy techniques to clients that can be incorporated into a home program; assist families in home program facilitation. Preform education of home program with client on land and in the pool.3.
With client input, develop individualized goals incorporating functional abilities with the objective to increase daily living skills. Measure and quantify client limitations and assist client in setting reasonable goals and expectations for their diagnosis.4. Develop and implement treatment program to meet the individual needs of the client.
Treatment may involve modalities, exercise, and aquatics in the plan of care. The goal is to maximize client independence in performing their exercises in the pool and on land to effectively carryover their plan to their daily life.5. Attend staff meetings, in-service trainings, and continuing education courses as appropriate or required.6.
Maintain communication with client's primary physician through progress reports or other communication measures.7. May participate in public speaking or community education programs that focus on issues relating to therapy and chronic pain/aquatic therapy. Will perform marketing of this adult program with the Director of Therapy Services to providers in the Green Bay and surrounding areas.8.
Assess and make recommendations regarding the use of therapeutic, adaptive, and/or positioning equipment as needed and provide written justification and coordinate purchase of equipment for a client with team members, service coordinator, intake/billing specialists, families, and other allied professionals.9. Fabricate adaptive equipment, splints/braces as indicated.10. Participate in evaluation staffings and may also be required to participate in case reviews, exit staffings, medical consultation staffings and employer meetins.11.
Responsible for timely documentation of evaluation results, progress reports, discharge reports, daily notes, attendance reports, Medicaid prior authorizations and insurance justifications as required.12. Provide clinical supervision to Physical Therapy Assistants.13. Maintain professional clinical competency by attending certified CEU in-service or workshops in the area of chronic pain and neurological conditions in aquatics.
Collaboratively incorporates best practices and demonstrate a willingness to share their knowledge with others. Provides in-service training to other members of the team.14. Assists in developing strategic and long-range plans for the program.
Works collaboratively with the Director of Therapy Services to promote, market, and grow this Adult Program in the Green Bay community.15. Meet productivity expectations.16. Performs any other duties that are assigned from time to time deemed necessary by the Director of Therapy Services.
