POSITION SUMMARY

Oversees the general and routine operation of the Adult Day Service program. Be responsible for Adult Day Service client case management. Prepares program reports for management review. Be responsible for ADS data system management. Works with Senior Wellness and Independence Manager on service quality improvement including Adult Day Service Policy and Procedure Manual update. Builds partnership with community stakeholders and markets Adult Day Service program.


DUTIES AND RESPONSIBILITIES

Oversees the General and Routine Operation of the Adult Day Service (ADS) Program:

  • Assigns and supervises ADS general and routine tasks to the Activity Assistant and Activity Supervisor.
  • Reviews and approves monthly activity calendars prepared by Activity Supervisor and ensures the activities are implemented based on funding sources’ requirements and Chinese American Service League’s policy.
  • Reviews and approves monthly Units of Services Calendar before submitting to the Operations Assistant to prepare the monthly billing vouchers to Accounting Department.
  • Provides monthly supervision to Operations Assistant and Activity Supervisor.
  • Supervises Activity Workers and Activity Assistants if needed.
  • Conducts annual performance appraisal with Operations Assistant and Activity Supervisor.
  • Reviews and works with staff on their goals and objectives yearly and keeps them on track.
  • Plans and schedules at least 24 hours of pre-service training for newly hired staff within the first week of employment and at least 12 hours of in-service training for employed staff each year and keeps documentation.
  • Provides annual training to the ADS staff to meet the requirements from the Managed Care Organizations (MCOs) and keeps documentation.
  • Facilitates monthly ADS team meetings.
  • Works closely with the Food Program Supervisor and contracted dietitian nutritionist to ensure the ADS food program is in compliance with the Child and Adult Care Food Program requirements.

Be responsible for Adult Day Service Client Case Management:

  • Conducts a thorough assessment within one month of attendance of a new client and a subsequent assessment of each client at not more than six month intervals.
  • Works closely with Nurse Coordinator and Activity Supervisor to develop individualized client care plans.
  • Supervises staff in implementing and monitoring each client’s Plan of Care.
  • Documents clients’ progress or changes or service interruption on recording sheets.
  • Participates in monthly Client Care Plan meeting and conducts reassessments.
  • Communicates with clients’ family/caregivers about the psycho-social changes of the clients and provides support to them if necessary.
  • Keeps close contact with Care Coordination Units (CCUs) and MCOs and makes sure that cases are handled appropriately.
  • Assists the Nurse Coordinator to communicate with clients’ physicians and the family/caregivers on the physical changes if necessary.
  • Documents and prepares reports for any incidents happened to the clients at Adult Day Services.
  • Provides clients with personal care and assistance such as toileting, laundry, bathing and eating.

Be responsible for service quality improvement:

  • Works closely with SWI Manager to ensure compliance with Illinois Department on Aging (IDoA) regulations and procedures and other requirements from different sources and accreditation.
  • Conducts client/family Satisfaction Survey and community agency survey annually.
  • Evaluates the program at least once a year, recommends changes, if needed, and ensures that the program is culturally and linguistically competent to the clients.
  • Reviews and updates the Operation Manual of Adult Day Services “ADS Policy and Procedures” at least every year as directed by IDOA and other funding sources’ rules and regulations.
  • Prepares files and documentation for internal and external audits.
  • Reviews and updates client’s and worker’s files as such a need arises.
  • Prepares program reports for management review.
  • Be responsible for ADS data management using Salesforce.
  • Builds partnership with community stakeholders and markets Adult Day Service program.
  • Participates in and contributes to department meetings, ADS team meetings, client care plan meetings, and staff training sessions.
  • Follows and implements ADS Policy and Procedures and Agency Policy on providing services to clients in ADS center.
  • Be responsible for own professional development, including participating in the supervisory process and using other educational opportunities, both within and outside of the agency.
  • Responds to agency needs by performing assigned tasks which do not fall within the above description. If such tasks are not of a temporary nature, they shall be added to this list of designed.
Requirements:

MINIMUM REQUIREMENTS

Education/Experience:

  • Bachelor’s degree in Social Work, Psychology or related health or human services field, with at least one year of supervisory experience.
  • Associate’s Degree in Social Work, Psychology, or related health or human services field, with at least 5 years of supervisory experience.
  • Two years Case Management experience, with a focus on elderly services desired.

Skills/Knowledge:

  • Spoken and written knowledge of Chinese and English. (Cantonese preferred, Toishanese and Mandarin a plus)
