This position is with the Management Service Organization for the Centers for Advanced Orthopaedics - MedVanta.

Position Summary/Scope of Responsibility

The Patient Care Navigator II will be responsible for providing guidance and education to patients and family members during the perioperative journey and/or the treatment plan. The Patient Care Navigator II will have advanced knowledge in the specialty of Orthopaedics and will serve as a single point of contact for patients and/or family members for the purpose of effectively communicating the patient's plan of care, and for coordinating related activities with the physicians and other members of the healthcare team. The Patient Care Navigator II will follow the care of the patient throughout the course of treatment and will actively engage with the patient throughout the episode of care so as to proactively ensure positive patient outcomes. The Patient Care Navigator II will assist with additional administrative and supervisory duties as assigned by the Senior Director of Patient Care Navigation.


Primary Responsibilities Include, But Are Not Limited To:

  1. Onboard and train new staff within the Patient Care Navigation Department.
  2. Oversee, maintain, and update staff schedules reporting weekly updates to the supervisor.
  3. Manage patient roster from the care management software, which includes the assignment of patients to the Patient Care Navigators.
  4. Maintain and update physician protocols list and contact preference file.
  5. Assist with administrative value-based contract duties which may include, but is not limited to, collecting and updating provider rosters and care partner agreements, analyzing claims data, and creating physician performance reports.
  6. Present pertinent value-based care information to stakeholders, clients, and/or potential clients on an as-needed basis as deemed by the supervisor.
  7. Provide guidance and education to patients and family members during the perioperative journey and/or treatment plan.
  8. Utilize interactive software to coordinate and evaluate patient care plans
  9. Monitor, report, and respond to patient deviations from clinical pathways to improve health outcomes and quality of care.
  10. Address patient issues, questions, and follow up telephonically and electronically, escalating to the physician/mid-level as necessary, serving as the tier 2 point of contact for those patients needing more enhanced patient care
  11. Serve as a point of contact/liaison for patients facilitating communication, referrals, and appointments amongst a multidisciplinary team.
  12. Document interactions and information in the electronic medical record and care coordination software.
  13. Performs other special projects and duties as assigned.
  14. Serves as a liaison between the surgeons, office staff, patients, hospital staff, and other multi- disciplinary team members.
  15. Interacts with surgeon's office staff and streamlines activities related to assigned patients.
  16. Collaborates with leaders of Rehab Therapy to ensure care is aligned with plan of care.

Required Education & Experience

  1. Registered Nurse with active, unrestricted Maryland or Compact/Multi-State License
  2. 6+years of clinical experience and/or experience relevant to the role as a registered nurse
  3. Strong experience in the orthopedic field
  4. Experience documenting patient information in Electronic Medical Records
  5. Experience creating and coordinating care plans, collaborating with a multi-disciplinary team.
  6. Experience & confidence providing patient education
  7. Telehealth and value-based care experience a plus
