Manages the Home Health clients and provides nursing support to home health nursing and personnel (clinical and clerical). In addition, this position will help with the supervision of the field staff.
EDUCATION and/or EXPERIENCE
Nursing diploma, unrestricted nursing license in the State of Florida; or one to two years related experience and/or training; or equivalent combination of education and experience.

Coordinate and case manage patient care services
Maintain updated patient records and perform record keeping including admission criteria, patient progress on narrative form in office chart, changes in medical orders and conditions, collaborative with other agencies for the benefit of the client.
Performs office tasks in conjunction with the Staffing Coordinator; such as staffing, referrals, filing, typing, data/computer entry, mailing etc.
Assures client plan of care (POC’s) is current and complete in accordance with Organizations policies and applicable laws.
Maintain on going quality assurance program with the DON by reviewing the following:
Case assignment and case management is appropriate, adequate and consistent with patient plan(s) of care, medical regime, and client needs.
Nursing services are consistent with professional community health nursing standards. Proper documentation of patient care is recorded.
Nursing and other services provided are coordinated
Services and outcomes are documented, dated and signed in the clinical service record.
Client infection(s) and complaint(s) are researched and resolved.
Confidentiality of patient date is maintained.
Findings of the quality assurance program are used to improve services.
Management of complaints or concerns received from home health staff and clients.
Provides nursing support to field professional.
Supervision of field staff.
Alternates “on-call” cellphone coverage with staffing coordinator as scheduled.
Assist Coordinator in the maintenance of schedule calendar for billing.
Completes clients Monthly Clinical Record review Guide and Monthly Clinical Review Sheet.
Reconcile all Home Health billing.

HIV & HIPPA Course
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from group of managers, clients, customers, and the general public.
