The social worker will help ensure a high quality of care in the health care facility by:
Assessing patients admitted to the health care unit.
Providing social work evaluations.
Lead interdisciplinary care plan meetings.
Formulating person-center care plans.
Instituting social work services and/or team interventions to facilitate adjustment, coping, or understanding.
Maintaining up-to-date documentation as mandated by regulatory bodies.
Securing resources for patients/residents on the health care unit.
Initiating groups or activities, either independently or in concert with other health care staff including activities staff, with the goal of therapeutically assisting residents and their families in adjusting to living at Broadmead.
Coordinate discharge plans for patients in the sub-acute rehab who are returning to their home at Broadmead, or to the community at large.
Identify patients/residents who may need referrals to psych services. Assist with coordination of referrals to psych services.
Provide in-services as needed to staff.
Serve on facility wide committees as requested.
Provide supervision to social work students as the opportunity arises.
Cover for the Director of Social work as needed.
Serve as the Coordinator of Care between hospice providers and the Broadmead community.
Conducts pre-residency interviews for potential Broadmead residents when director is unavailable.
Be aware of HIPAA security regulations.
The social worker will help ensure a high quality of life among Broadmead residents by being available for therapeutic counseling on a variety of issues including, but not limited to:
Adjustment to institutional living.
