Basic Job Duties
Provide community support and care coordination services for a caseload of individuals with mental illness who are participating in the Supported Employment Demonstration (SED) project sponsored by the Social Security Administration. Collaborate with the Full Service and Basic Service SED teams to coordinate services for individuals being served. Assess consumer’s overall functioning, identifying problems, needs, skill strengths and deficits, and assumes a care coordination role in the consumer centered planning process.
Bachelor's degree (B.A./B.S.) from four-year college or university in the human services field and prefer at least one year of direct care experience working with persons with disabilities. Must have Mental Health Professional (MHP) status to work with persons with mental illness. Must be able to achieve Qualified Intellectual Disability Professional (QIDP) status to case manage persons with developmental disabilities.
Must be able to communicate respectfully and effectively in both oral and written forms; must have good social and assessment skills; must have the ability to work independently and interact appropriately within the public domain.
Must have reliable transportation, valid driver's license, proof of insurance, and be insurable. Also must obtain and maintain Blood Borne Pathogens, OIG, Corporate Compliance and Security Rules. CPR and First Aid training after hired.
