    • POSITION SUMMARY
      • The Pre-Visit Planning nurse assists clinic providers and staff in the deliverance and coordination of patient care, in order to facilitate patient satisfaction, clinic efficiency, and provider productivity. The Pre-Visit Planning Nurse reports to the Primary Care Nurse Manager. This nurse promotes the mission, vision and values of Bozeman Health in the ambulatory setting and is able to be direct and truthful, while maintaining trust and confidence. They provide compassionate and person-centered care across the continuum focusing on patients and families, providers and the entire care team. This nurse provides care excellence with adherence to clinical pathways and protocols and promotes of culture of continuous improvement with suggestions on efficiency and effectiveness of operations. The Nurse Practices Act and Administrative Rules of the state of Montana govern Registered Nursing practice.
  • REQUIREMENTS
    • EDUCATION
      • Required:
          • Associate degree, or Diploma from accredited School of Nursing
      • Preferred:
          • Bachelor's degree form accredited School of Nursing.

    • LICENSURE and CERTIFICATION
      • Required:
          • Current Montana RN license, in good standing
          • Current CPR certification
      • Preferred:
          • N/A

    • EXPERIENCE
      • Required:
          • 2 years clinic or acute care nursing experience
      • Preferred:
          • Computer ability and knowledge of electronic health record operating systems
          • Ambulatory care clinic experience
          • Experience with Epic EHR and NextGen.

    • KNOWLEDGE OF,  SKILLS IN, ABILITY TO, COMPLEXITY AND DIFFICULY:
      • KNOWLEDGE OF:
          • Basic pathophysiology as it relates to acute and chronic disease processes.
          • Various diagnoses and treatments of disease states and knowledge of wellness/health maintenance.
          • Factors related to safety, effectiveness and cost planning when delivering care to patients.
      • SKILLS IN:
          • Paying close attention to detail in providing patient care.
          • Customer service skills including interpersonal, verbal and written communication skills.
          • Utilizing computer skills-hardware basics and software programs including but not limited to e-mail, word processing and spreadsheets.
          • Efficient use of electronic health records with accurate abstraction to patient's Bozeman Health record as required.
          • Documenting in the medical record all phone conversations, verbal instructions and other items discussed with patients, their families and other individuals involved in the patient's care.
      • ABILITY TO:
          • Demonstrate initiative in performing responsibilities and working independently with a minimum of supervision.
          • Utilize organizational skills, maintain flexibility to adapt to work assignment and possess excellent time management skills.
          • Follow policy manuals for established policies and procedures, specific instructions, and guidelines including HIPPA.
          • Assist patient in navigating through the clinic in friendly, professional and efficient way.
      • COMPLEXITY AND DIFFICULTY:
          • Collaborate with community providers to coordinate patient care.
          • Supports and practices "Commitment to a Culture of Excellence."
  • ESSENTIAL FUNCTIONS
    • 15% Time Spent -

      Coordinates with clerical and clinical staff to ensure scheduled patients' medical record is as complete as possible prior to the patient’s appointment.

    • 20% Time Spent -

      Contact patient via telephone for pre-visit planning. This call would include clarifying the patient's agenda, anticipating any special needs as well as, obtaining information regarding past medical and surgical history, family history and social history, health maintenance screenings, immunizations, advance directive documents and care team providers.

    • 20% Time Spent -

      Review patient's current medications and allergies. Contact the patient's pharmacy for accurate prescription medication dosages if time allows.

    • 20% Time Spent -

      Update/abstract patient's health maintenance screenings as appropriate from medical records.

    • 20% Time Spent -

      Document information obtained in patient encounter in the medical record accurately, appropriately, and completely.
