Status: Part-time, 40 hrs per pay period




Shift: Tuesday and Friday, and every other Wednesday, 8am-4:30pm; Will participate in after hours call rotation




Position Summary:




The Nurse Practitioner provides patient care services by evaluating, treating, and triaging patient needs within the scope of the individual authority granted by the applicable governing Idaho State Board. The Hospice Nurse Practitioner ensures the delivery of individualized nursing care to Hospice clients, focusing on providing highest quality of life and palliative care.




ESSENTIAL FUNCTIONS:







+ Provides Hospice Admission visits and Medicare regulation Required Face to Face Encounter visits as well as Nursing Assessment visits




+ Communicates assessment findings to Hospice Medical Director for determination of eligibility for initial and ongoing Hospice services




+ Writes appropriate orders for Hospice admission, services, treatments, medications as well as ongoing care




+ Acts as Hospice Attending Provider for Hospice patients who do not have community provider to oversee care




+ Provides clinical oversight and guidance to Hospice team in patient care management needs




+ Provides nursing care on an intermittent basis for care and symptom management




+ Participates in standby coverage to provide 24-hour nursing availability to Hospice clients; scheduled for standby after hour duties as department needs require to meet patient care needs (a minimum of once every four week nights)




+ Participates as a member of the Interdisciplinary Team (IDT) to assist in provision of holistic care to clients and families




+ Documents all Face to Face encounters, Orders, clinical findings, and any intervention provided by end of each scheduled shift




+ Consults with client's attending physician and/or Hospice Medical Director when necessary regarding symptom management




+ Participates in standby coverage to provide 24-hour nursing availability to Hospice clients; scheduled for standby after hour duties as department needs require to meet patient care needs (a minimum of once every four week nights)




+ Maintains productivity standards as defined by Home Health/Hospice Manager or Designee




+ Demonstrates the ability to communicate and receive constructive feedback in a positive manner




+ Provides in-home education to promote management of Hospice patient need maximizing independence, and holistic care safe activities of the client




+ Monitor client and caregiver's progress toward stated goals. Collaborates with Hospice team to adjust POC according to client response





Requirements
Skills





+ Ability to provide professional care based on training, competence and judgment




+ Must have full capacity to read, write and comprehend as well as full capacity to make complex and routine medical judgments




+ Must have contemporary skills and documented CME's




+ Basic computer literacy and ability to effectively learn new software programs required





Education





+ Graduate from an approved professional school of nursing




+ Hospice training as provided by BGH and a professional interest in working with the terminally ill and their families





Experience:





+ Documented evidence of a minimum two years professional experience and ability in direct patient care




+ Medical/Surgical experience preferred





Licensure/Certification:





+ Current State of Idaho Nursing Practitioner License and certification through the applicable accrediting organization.




+ Meets and maintains credentialing requirements of Allied Health category of BGH staff affiliation




+ Current BLS (AHA) certification




+ Valid driver's license
