The Care Management Manager is responsible for overseeing an assigned group of Lead Care Managers, Post-Acute and Behavioral Health Care Managers and Operations staff throughout our Midwest Markets (MN/WI). This position provides regular supervision, mentorship, support, guidance, and education to these Lead Care Managers, Post-Acute and Behavioral Health Care Managers and Operations staff. The Care Management Manager will provide 1:1 individual meetings to Lead Care Managers, assist in coordinating regularly assigned group meetings, performance reviews for Lead Care Managers and participate in Leadership development meetings.

Primary Responsibilities:

  • Organize and assist in the interviewing, training and educating of Lead Care Managers and Post-Acute and Behavioral Health Care Managers
  • Provide supervision and support to Care Management Leads and staff members
  • Oversee/coordinate/facilitate/plan/participate in meetings for Care Management and BPS as needed
  • Disseminate department updates
  • Relationship building with regional partners
  • Organizing and participating in community outreach events
  • Monitor team operational (Post-Acute and Behavioral Health) and financial performance (Behavioral Health)
  • Become an expert in Bluestone technology (EMR, Bluestone Bridge, etc.) and communication platforms/hardware (EMR, mobile devices and apps, software)
  • Assists others on the team to develop competence when communicating with internal and external stakeholders
  • Provide coaching, corrective action, and disciplinary action to direct reports
  • Provide monthly check-ins and annual reviews to direct reports
  • System development participation (i.e., training, education, efficiencies, process improvement, etc.)
  • Policy and Procedure development
  • Attend all staff meetings, ongoing education opportunities and other collaborative meetings
  • Provide ongoing project support as needed


  • Bachelor’s degree in nursing, social work, health care administration or other related field.
  • Care management experience
  • Management/leadership experience strongly preferred
  • 3-5 years of experience working in care management or closely related field
  • Experience developing and creating training and policy/procedure content preferred
  • Proficient presentation, written, and verbal communication skills 
  • Strong attention to detail, follow-through, and commitment to quality 
  • Excellent technical, interpersonal, and customer service skills 
  • Skill in identifying and resolving problems 
  • Creative problem solving skills
  • Ability to lead others in an effective, positive manner
  • Ability to work with diverse populations
  • Must have a valid driver’s license; approximately 25% travel is required
  • Ability to work independently to meet department and company goals and timelines 
  • Ability to deal with change and ambiguity 
  • Detail oriented and accurate 
  • Computer proficiency including Google Docs, EHR and equipment such as iPad, and iPhone 
  • Demonstrated compatibility with Bluestone’s mission and operating philosophies 
  • Demonstrated ability to read, write, speak, and understand the English language
