HIV Case Manager/Care Coordinator

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Under the supervision of the Program Director the HIV Case Manager/Care Coordinator will be responsible for the following:

  • Manage HIV/STD counseling and testing activities, including logistical and administrative
  • Elements of outreach and special events related to HIV/STD testing and counseling
  • Organize ongoing patient outreach activities, including establishment and maintenance of liaison ties with all HIV/STD-related external service and government agencies (including funding agencies)
  • Facilitate referrals, create and maintain interdepartmental communication and service linkages to other organizations to promote comprehensive care for HIV and high risk patients
  • Health Care Plan objectives/create a Service plan with goals and objectives should be S.M.A.R.T ( Specific, Measurable, Achievable, Realistic and Time-Bound according with the patient’s assessment/reassessment.
  • Administers Modified Directly Observed Therapy(mDOT)
  • Reporting data in eSHARE
  • Participate and follow PCMH guidelines
  • Participate in all HIV/STD Care Team meetings
  • Facilitate Health Education groups and trainings
  • Create and maintain HIV care continuous quality assurance measures, in consultation with Infectious Disease Specialist and Program Director
  • Create and maintain PrEP/PEP care continues quality assurance measures according with guidelines
  • Monitor and report grant deliverables on a monthly basis for the HIV, STD, HCV Prevention, PrEP/PEP, and Related Services for patients in the HIV program
  • Leads comprehensive orientation, assessment and care planning activities as clients are enrolled
  • Verifies eligibility, ensures services are not duplicated by another provider
  • Supervises and collaborates with patient navigators on ongoing assessment activities, planning and service delivery
  • Coordinates with medical providers to incorporate the latest medical treatment plan into the client service plan and other services that the client might need, facilitates interdisciplinary case conferences with HIV care team and other service providers.

KNOWLEDGE, EDUCATION, SKILLS AND ABILITES REQUIRED:

  • The Case Manager/ Care Coordinator has over two years of experience in case management with the HIV/AIDS population
  • One year of relevant experience providing services to vulnerable populations and LGBT community
  • Bachelor in Human services or related fields or two years of experience providing Case manager services to vulnerable population
  • Self-directed with the ability to prioritize multiple tasks
  • Knowledge of two evidenced based prevention and interventions for Vulnerable populations
  • Demonstrated understanding of the bio-psychosocial and cultural dimensions of HIV disease, STDs, and its impact on minority, underserved, populations
  • Broad knowledge of the political, systemic, administrative and organizational complexities of HIVISTDs care
  • Knowledge of Biomedical tools such as PrEP and PEP
  • Demonstrated understanding of community health care service delivery systems and their role in providing HIV/STD care services
  • Strong analytical, interpersonal and communication skills (verbal and writing)
  • Knowledge of database programs and Microsoft Office
