Maternal and Child Health Project Director

BPN currently operates an innovative maternal health program with dynamic community partners to enhance connections across health and community sectors for the maternal-child health population. The focus of this program is to identify and serve high-risk pregnant mothers in OB/GYN practices and 0-3-year-old Medicaid managed care patients found in pediatric practices and screen these individuals for socio-emotional risk factors. This screening will make a determination of needs and coordinate appropriate referrals and linkages to community-based resources to support those needs. The initiative aims to improve the physical, social, and emotional needs of children and enhance their school readiness, improve infant and maternal mortality by supporting the mother during her pregnancy, and reduce barriers to appropriate services.
The chosen candidate for this position will assume overall directive and operational responsibility for providing guidance and direction for all team members. The Project Director will supervise a team that is responsible for improving behavioral health and social determinants/instrumental needs outcomes for community clients. This team will include a LCSW Clinical Supervisor, and LMSW Supervisor, and a Quality team. We are looking for a self-motivated clinical and administrative professional with behavioral health expertise who has a passion for and experience in reducing social determinants of health.

POSITION DESCRIPTION:
The Project Director is a key decision-maker for maternal and child health programs. Reports to the Brooklyn Perinatal Network (BPN) Executive Director, and supports a project advisory committee, as well as the Executive Director in reporting to the BPN Fiduciary Board.
The project is a Community-Based Organization (CBO) clinical partnership utilizing LicensedMaster Social Workers (LMSW), Licensed Mental Health Counselors (LMHC), Community Health Workers (CHW) as the core service delivery staff. The Project Director will provide leadership for overall project development and implementation working as a team with the clinical and community partners to accomplish the goals of initiative reporting to the Department of Health (DOH); and engaging with Health Plans to develop buy-in for sustainability of the project work.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The Director will oversee and assure:
· Project development, implementation, and evaluation; and provides overall direction of implementation for the project, supervising all project Staff.
· Development and effective utilization of operational policies and procedures and compliance monitoring
· Compliant data tracking procedures that will include - project implementation in compliance with the required NYS tracking procedures, IT system implementation and required client data collection, analysis, reporting as relevant and required by the project model
· Quality improvement practices by directly supervising the Quality Assurance/Data Manager to ensure the utilization of developed quality improvement principles in all program operations including ongoing quarterly data collection (summary level data and individual data) using PDSA principles
· Evidence-based community grounded clinical services through the supervision of the LMSW Clinical Supervisor
· Development and implementation of brief, integrated behavioral health services that provide stabilization and referral to follow-up care in collaboration and supervision of the LCSW Clinical Supervisor.
· Development of contracts/BAAs/MOUs with partner agencies
· Preparation of monthly narrative reports with review and submission of monthly NYS DOH Referral Trackers to NYS DOH maternal and child health project managers.
· Monthly program reports for evaluation by the Executive Director and ongoing release of project-related information
· Client Satisfaction Survey and result dissemination
· Ongoing alignment and use of NYC Resource Directory implementation
DESIRED MINIMUM QUALIFICATIONS:
· Master-level degree and training, graduating with MPH, MBA, LMSW or other relevant
areas of health
· Five plus years of experience with implementing community partnership initiatives
· Advanced understanding of the Evidenced-Based Community Pathways Program Model
· Five plus years of experience working in a leadership role responsible for service
delivery in a community-based organization, preferably in NYC.
· Five plus years serving multi-ethnic/multicultural population in underserved
communities providing social health services
· Significant experience addressing maternal and child needs and addressing social
as well as an understanding of health disparities and inequities\
· Successful supervisory experience
· Excellent organization and project management skills
· Excellent communication skills
· Excellent computer application skills, particularly Microsoft Outlook, Word and Excel
· Ability to learn, apply, and implement new computer technology and/or databases
· Capacity and desire to work in a diverse, multicultural environment and community, with strong intention to eliminate the disparities affecting the black and brown communities in central Brooklyn
· Ability to build trusting relationships and work collaboratively in a dynamic, fast-paced environment

We are launching a pilot program with SUNY Downstate under NY State’s First 1000 Days Maternal and Infant Care Initiative to enhance connections across health and community sectors for the maternal-child health population. The focus of this pilot demonstration is to identify and serve high-risk pregnant mothers in OB/GYN practices and 0-3-year-old Medicaid managed care patients found in pediatric practices and screen these individuals for socio-emotional risk factors to make a determination of needs and coordinate appropriate referrals and linkages to community-based resources to support those needs. The aim is to improve the physical, social, and emotional needs of children and enhance their school readiness, improve infant and maternal mortality by supporting the mother during her pregnancy, and reduce barriers to appropriate services.
The chosen candidate will work within the community primarily to address client needs and support linkage to resources and address immediate needs until other resources are accessed, but also, CHWs will make weekly visits to the Downstate Campus for engagement with BPN’s clients.
The ideal candidate is someone with experience in case management/care coordination and the enthusiasm and vision for participating in strategically growing a program, willing to engage their professional skills, knowledge, experience and passion in a creative setting, and a strong collaborator who would form working relationships with BPN’s core network partners.
