BAYADA Hospice is looking for a compassionate and dedicated Nurse Practitioner to join our Fayetteville Hospice team per diem.

This office services hospice clients on a per visit basis in territories throughout Bladen, Cumberland, Harnett, Hoke, Moore, Robeson, and Sampson Counties.

Prior Hospice experience is strongly preferred.

As a hospice NP, you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

• You will be working with the office team to schedule and complete face to face visits, support with referral/admission review for eligibility and orders.
• 2 weeks/month rotation of on call to support the medical director, attend IDG meetings as appropriate, all staff meetings and training.

Nurse Practitioner (NP) Responsibilities:

  • Follow a designated care plan in accordance with patient's needs
  • Make home visits to clients in designated geographic territories
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures
  • Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
  • Follow up with, execute, and properly document doctors' orders
  • Perform client assessments as necessary
  • Case management and coordination
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Nurse Practitioner (NP):

  • A current license as a Nurse Practitioner in the state of North Carolina.
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.
