Physical Demands and Working Conditions:
Work is normally performed in a typical office work environment. While performing the responsibilities of this position, the individual is required to have ambulatory skills sufficient to visit various parts of the organization. Requires the ability to reach with hands and arms, climb stairs and occasionally lift and/or move up to 30lbs. Requires sitting and standing for extensive periods of time, operating a computer keyboard, telephone and other equipment for extended periods of time. Travel to other Axis health centers and other local travel will be required. ESSENTIAL DUTIES/RESPONSIBILITIES:
This position serves as a hybrid role helping to complete both Quality Enhancement and Information Systems initiatives and tasks under the direction of the Senior Director of Quality Enhancement
Quality Enhancement
- Disseminate data retrieved from medical record reviews, reports, and surveys to improve patient health outcomes
- Coordinate internal mock audits in preparation of regulatory site reviews, working collaboratively with all Axis Departments to ensure compliance
- Generate on-going monthly reports that monitor quality performance including, but not limited to UDS, HEDIS, and operational efficiencies
- Generate and follow-up on monthly reports that are used to operationalize the Care Based Incentive Program
- Coordinate and help administer/maintain Quality & I.S. SharePoint site
- Coordinate Peer Review, with direction from the Chief Medical Officer
- Collect and analyze and present aggregate trends on patient complaint and incident report data
- Coordinate CHCN audits
- Dictate and complete meeting minutes for routine committee and project meetings
- Assist in implementing quality improvement initiatives and PDSA activities
- Assist Data Quality Analyst in developing and validating reports extracting requested information from database systems
- Coordinate patient satisfaction survey and patient outreach efforts
- Analyze reports and help make recommendations for interventions to improve performance
- Coordinate Quality Enhancement Pilot Projects with the Community Health Center Network and Alameda Health Consortium
- Attend meetings and trainings, as needed
- Other duties as assigned
Information Systems
- Address tier 1 I.S. helpdesk tickets (i.e. EPIC password resets, MyChart Enrollment, FYI flag, proxy accounts, duplicate accounts)
- Coordinate I.S. training program which includes Epic user orientation, ongoing training, and training materials
- Maintain and update Axis EHR workflows
- Attend relevant OCHIN meetings and trainings
- Other duties as assigned
REQUIRED COMPETENCIES:
- An aptitude for information systems and utilizing quality improvement methods to improve patient outcomes
- Skilled in Microsoft Office Suite, specifically PowerPoint, Excel and Word
- Knowledge of the Plan-Study-Do-Act (PDSA), or deeming model for improvement
- Knowledge and experience in SharePoint and Microsoft Teams preferred
- Knowledge and experience with Tableau preferred
- Knowledge of EHR/EPM (EPIC) preferred
- Experience with SQL and solid understanding of a range of query tools such as Crystal Reports, Business Objects, Clarity, Tableau, etc.
- Excellent note-taking, dictation, writing, business communication, editing, and proofreading skills.
- Ability to plan, organize and lead data collection activities
- Ability to present statistical and technical data in a clear and understandable manner utilizing appropriate visual aids
- Must be organized and able to work with minimum supervision
- Excellent time management skills to meet goals and objectives within deadlines
- Must be professional and able to communicate sensitive or confidential issues with the utmost discretion
- Demonstrated enthusiasm and drive to build internal and external partnerships while promoting Axis in a professional manner
- Proactive, self-motivated and able to work independently as well as on a team with the ability to exercise sound independent judgment.
- Ability to type 45/wpm with minimal errors.
- Ability to interact effectively and in a supportive manner with persons of all backgrounds.
QUALIFICATIONS:
- Bachelor's degree in related field with two years of experience.
