SUMMARY. The Client Medication Coordinator is a role model that performs duties and responsibilities for client's safety, care, social & service support and development and oversees the medication for clients of the residential units. This position ensures that all the client's physical health needs are met.
This position is supervised by the Executive Director and is a part-time position.WORK HOURS: The incumbent may be assigned to various shifts.ESSENTIAL FUNCTIONS: Properly logs in all client medications. This includes checking labels for accuracy, completing proper documentation and storing medications as per AV and state policies Makes copies of all prescriptions, updates client charts as well as the medication and treatment record when new mediations or change orders are prescribed Review and monitor regular reporting cycles and interpreting that data to drive actions with external medical staff, clients & management. Ensures effective systems are in place for administration & ordering of medication to clients Processes medications for new residents and helps coordinates schedules and treatment records Oversees medications that are taken at school and on visits Acts as a liaison with resident, doctors, pharmacies, providers, and all residential staff as necessary to ensure timely and accurate ordering of medications and appointment scheduling as needed Oversees the supervision of all residents' medications in accordance with physician's orders, agency and state policies Reviews the Medication Administration Record (MAR) daily to ensure accuracy of the supervision of the medications Reports significant issues regarding client medications to a management team member (i.e. missed does of medications, resident refusals, adverse reactions, etc.) Assist with orientation of all new applicable staff with regard to proper supervision of resident medications Responsible for the monitoring and documentation of all residents involved in the self-medication process Respects and maintains resident confidentiality and demonstrates a caring, positive attitude towards all residents, family members, staff, guests and service providers Ensures the medication room is clean, organized, quiet and a welcoming environment for residents Performs any other duties necessary for the effective operation of the program and the medication room in particular Administrative tasks including weekly reporting on medication discrepancies, dose changes and discontinued medications for clients Performs other duties assigned by Executive Director.PHYSICAL DEMANDS: Frequently move/lift between 25-50 pounds, depending on assignment requirements Perform all duties that may require sitting, kneeling, squatting and walking Manual dexterity and fine motor coordinationREQUIRED EDUCATION AND EXPERIENCE: Candidates must be at least 21 years of age or older High school diploma required 2 years minimum experience as a CMA or Pharmacy Technician, LPN preferred Proficient technology skills utilizing either a Macintosh or Windows platform, including the Microsoft Office suite (Word, Excel, Outlook, PowerPoint) requiredDESIRABLE KNOWLEDGE, SKILLS, and ABILITIES: Current CPR/First Aid Certification Knowledgeable and sensitive in working with culturally diverse staff and clients Excellent interpersonal and communication skills, including both oral and written.
