Aurora provides residential and vocational services to individuals with brain injuries, physical disabilities, cognitive disabilities and/or mental illness.Aurora Residential Alternatives Regional Crisis Stabilization Program assists individuals who are experiencing a mental health crisis, providing comprehensive services in a community-based setting. Aurora's skilled mental health technicians support individuals with a wide range of emotional and mental health issuesPosition Summary: This position is responsible for providing assistance and training to consumer(s) in order to meet identified needs in a safe environment, assuring delivery of consistent and quality services to consumers, guardians and the contracting agency.Consumer Advocacy Assist/teach skills that create independence and involvement in all aspects of life Utilize consumer strengths to build new skills, achieve goals, and enhance quality of independent living Suggest changes to improve consumer lifestyleMedication/Health/MAPC Follow ISP/Crisis Plan/Safety Plan Ensure day-to-day compliance with Adult Family Homes (AFH) Accompany consumers to scheduled medical appointments and communicate order changes in medication, treatment orders, and therapies Provide medication administration assistance as dictated by consumer program plan Complete/follow consumer daily menu, following prescribed special diets Report changes in consumer's medical condition or eating habits immediately to supervisor Provide basic first aid such as controlling bleeding, caring for shock, burns, muscle, bone, and joint injuries, follow poison control procedures, and contact EMS (911) as necessary Implement procedures for infection control in compliance with Infection Control Policy and Exposure Control Plan Ensure safe work practices Perform other duties as assignedDocumentation/Communication Use strict confidentiality practices at all times Participate in intake/admission/discharge of consumers Maintain complete and accurate records of medical documentation, consumer's progress, change in condition, and activities during each shift Create and oversee the day-to-day implementation of the consumer service plan and program components Maintain current consumer files, policy manuals, and additional supportive documentation Serve as the primary contact/coordinator for all inter-agency programming e.g. medical, counseling, vocational, etc Maintain regular contact with team members, consumers, guardians, case manager, and supervisors to ensure the quality and continuity of consumer care Participate in intake/admission/discharge of consumers Complete consumer/operational records reviews Conduct thorough onsite training with new team members that demonstrates accurate procedures Attend all agency sponsored training's and staff/weekly supervision meetings Follow through on noted concerns/requests Maintain consumer /site receipts and ledgers, and complete shift audits Assist individuals with spending needs and comply with operational budgets/purchasing guidelines Provide guidance, assistance, or training to others where appropriate Interact with others in a positive, cooperative, and effective manner Maintain professionalism when responding to crisis situations and challenging behaviors Perform other duties as assignedRequirements/Qualifications Experience supporting individuals with a variety of disabilities include those with challenging behavior's 18 years of age Acceptable Background Character Verification Valid drivers license and acceptable driving record for consumer transport Ability to lift up to 50 lbs.
