Home Care Physical Therapist

We have immediate openings for Physical Therapists for home visits in Brooklyn! NYS license as a physical therapist is required. We are an established facility that offers excellent rates, flexible scheduling, and biweekly pay. The patient population includes geriatrics and/or pediatric patients to be seen in their home environment. Friendly staff, communication via text/ WhatsApp during the day, and a family-like work atmosphere. This position will be per-diem to start and will lead to part-time and possibly full-time as our caseloads grow.

Responsibilities

• Evaluates patient needs for Physical Therapy
• Completes initial and ongoing assessments to determine patient level of functioning
• Assess environmental factors that effects patients care and treatment plan
• Revises plan of care based on patient’s abilities and needs in consultation with the patient care team
• Complete documentation using online EMR in a timely manner
• Obtain vitals as needed
• Familar with stretching, soft tissue mobilizations, myofascial release, manual therapy and corrective exercises

Requirements:

• NYS PT License
• Current NYS PT Registration
• Malpractice Insurance
• Valid Driver’s License

Salary:  $75.00 per hour
